This project consists of a single contract for the construction of the Warrick Trails - Anderson Road (Vann to Amax). Warrick Wellness Pathways, Inc. shall be referred to and considered the Owner in this Project Manual, and will manage and oversee the construction of the Project. Any Bid(s) received after the designated time for bid opening, for any reason, will be returned unopened. All Bid(s) must be delivered in a sealed envelope bearing the name and address of the Bidder, indicating identification of the Project. Each Bid(s) must be submitted on State Board of Accounts Form 96 (revised 2013) for public works projects. Part I and Part II, Sections I, II, III, IV and V shall be fully completed by all Bidders. Bidders may submit General Form 96a Standard Questionnaires and Financial Statement for Bidders, Standard Questionnaires and Financial Statement for BiddersForm 102, or a Bidder's current Financial Statement by a Certified Accountant to comply with Part II, Section III. Bid(s) shall not be withdrawn after the opening of Bids for a period of sixty (60) days after the scheduled time of closing Bid(s) without the consent of Warrick Wellness Pathways, Inc. Contractor shall furnish an approved Ten Percent (10%) Bid Bond with his/her Bid if the bid amount is in excess of $200,000. Contractor receiving award will be required to furnish an approved 100% Performance Bond and Payment Bond if his/her bid is in excess of $200,000. Said bonds shall remain in effect for twelve (12) months from the date of acceptance of work. Construction and installations shall be in full accordance with the Drawings and Specifications, together with all Addenda thereto. Warrick Wellness Pathways, Inc. reserves the right to reject any and/or all Bids, and to waive any informality in bidding. The Work is not subject to the Common Construction Wage Determination. Failure to attend shall render any Bid non-responsive. Contractor must enter into an agreement with Warrick Wellness Pathways, Inc. that the Contractor enroll in and verify work eligibility status of all newly hired employees through the 'E-Verify Program' as stated in Indiana Code 22-5-1.7.
