On behalf of the Memphis VA Medical Center, Network Contracting Office (NCO) 9 is issuing this sources sought notice (SSN) as a means of conducting market research to identify parties having an interest in, and the resources to support, this requirement for a firm-fixed priced contract for the Memphis VA Medical Center Sound System Upgrade. The result of this market research will contribute to determining the method of procurement. The applicable North American Industry Classification System (NAICS) code assigned to this procurement is 334310 Audio and Video Equipment Manufacturing. THERE IS NO SOLICITATION AT THIS TIME. This request for capability information does not constitute a request for bids, proposals, or quotes; submission of any information in response to this market survey is purely voluntary; and the government assumes no financial responsibility for any costs incurred. If your organization has the potential capacity to perform these services, please provide the following information: Organization name, address, point of contact, email address, web site address, and telephone number. Business size: small, small disadvantaged, woman-owned, Hub Zone, serviced-disabled veteran-owned small business (SDVOSB), veteran owned small business (VOSB), large business, etc. Business type under NAICS 334310, which contains a size standard of 750 employees, Commercial And Government Entity (CAGE) Code and Unique Entity Identification (UEI) Number. Under which NAICS code does your company usually provide the requirements described in the Statement of Work (SOW) - Please provide rationale for your answer if different from NAICS 334310. Tailored capability statements addressing the particulars of this effort, with appropriate documentation supporting claims of organizational, and staff capability. Please provide the following information as an attachment to your response: Identify any other companies owned (wholly or in-part) by the owners of this business who provide goods or services under the same or a related NAICS codes. Response to this SSN shall not exceed 10 pages and should include all of the fore-mentioned information. The government will evaluate market information to ascertain potential market capacity to: Provide services consistent in scope and scale with those described in this notice and otherwise anticipated. Secure and apply the full range of corporate financial, human capital, and technical resources required to successfully perform similar requirements. Implement a successful plan that includes: compliance with program schedules; cost containment; meeting and tracking performance. Provide services under a firm-fixed-price contract. NOTE: Respondent claiming SDVOSB and VOSB status shall be registered and Center for Veterans Enterprise (CVE) verified in VetBiz Registry www.vetbiz.gov. Based on the responses to this SSN/market research, this requirement may be set-aside for SDVOSBs, VOSBs, small businesses, procured through full and open competition, or sole sourced. The government reserves the right to use any non-proprietary technical information in any resultant solicitation(s). See attached SOW. STATEMENT OF WORK Memphis VA Medical Center Sound System Upgrade SCOPE OF WORK The contractor shall provide all services, labor, travel, transportation, material, shipping, parts, and equipment necessary to perform repair for the equipment listed in the Equipment section of this Statement of Work in accordance with the original manufacturer s specifications for the Lt. Col. Luke Weathers, Jr. VA Medical Center located at 1030 Jefferson Avenue, Memphis, TN 38104, to ensure dependable and reliable equipment operation. EQUIPMENT See below for equipment. REQUIREMENTS Theater Install a new ceiling-mounted projector. The projector will be brighter at 13,000 Lumens. Replace the existing screen with a new electric screen. The system will include two (2) HDMI wall plate inputs. The system will have three (3) cameras and two (2) suspended microphones for Conferencing. The existing pendant speakers in the Main Section will be replaced and a new speaker zone will be created for the lower ceiling section at the rear. The new zone will consist of ceiling tile speakers. Conferencing connections will be via an USB connection at the DSP and at one (1) wall plate location. Full Control will be provided via Q-SYS control with a 7 touch panel. Evacuate any non-critical existing cabling during the installation of the new system. Additional: The option for upgrading the wireless microphone systems to include three (3) single-channel wireless receivers with a combination of handheld or lapel microphone transmitters. D-106 and G-140 Conference Rooms Install a 75 display on a tilting wall mount. The wall mount will include the Pull-out feature for service. The system will include Two (2) HDMI wall plate inputs. Utilize the existing Cameras but will install a ceiling mounted microphone for Conferencing. The existing amplifier and speakers will be integrated for use with the new system. Conferencing connections will be via an USB connection at the DSP and at one (1) wall plate location. Full Control will be provided via Q-SYS control with a 7 touch panel. G-154 Conference Rooms Install a 75 display on an articulating mount. One (1) HDMI Input and One (1) USB connection will be available at the display. Install a soundbar below the display. The soundbar will have a built-in camera along with mic and speaker functionality. Paging Install a new headend for paging. This will consist of one (1) new desk microphone, one (1) matrix DSP, and one (1) amplifier. Connect the new headend into the existing speaker legs. SAFETY The contractor/vendor shall always, ensure that all work provided complies with all local, State, and Federal rules and regulations pertaining to workplace safety. The contractor shall take all proper safety and health precautions to protect the work, the workers, the public and the property. DOCUMENTATION N/A PLACE OF PERFORMANCE Lt. Col. Luke Weathers, Jr. VA Medical Center; 1030 Jefferson Avenue, Memphis, TN 38104 HOURS OF OPERATION The contractor will perform services required during the hours of 8:00 am 4:30 pm, Monday through Friday or as directed by the designated Lt. Col. Luke Weathers, Jr. VA Medical Center Point of Contact. U.S LEGAL HOLIDAYS The following is a list of U.S. Government holidays. If the holiday falls on a Saturday, the proceeding Friday is observed as the holiday; if the holiday falls on a Sunday, the following Monday is observed as the holiday. Employees must have a valid state or government identification. All contractor/vendor personnel shall display a Lt. Col. Luke Weathers, Jr. VA Medical Center issued identification badge while performing work on any VAMC property. Smoking is prohibited on property except in designated areas. Possession of weapons is prohibited (in vehicle or on person). Enclosed containers, including tool kits, shall be subject to search. Violations of the VA regulations may result in citation answerable in the United States (Federal) District Court. Contractor employees (crew) when on VAMC Memphis grounds; shall not bring visitors into the facility; shall not bring into the facility, any form of weapons or contraband; shall not bring in any alcohol or drugs or be under the influence of alcohol/drugs; shall conduct themselves in a professional and polite manner at all times; shall not cause any disturbances in the facility, are subject to all the rules and regulations of the Lt. Col. Luke Weathers, Jr. VA Medical Center. Contractor personnel shall present a neat appearance and be easily recognized. Contractor shall furnish and require every on-site company employee to wear a company shirt and safety shoes. All company workers while on duty shall be identified by uniform shirt or blouse indicating the company logo. The print should be large enough to be easily read. In addition, all company staff assigned to work at the VAMC Memphis. SITE REGULATIONS: All work shall be performed in accordance with Lt. Col. Luke Weathers, Jr. VA Medical Center rules & regulations. CONTRACT TYPE: Firm-Fixed Price One Time Purchase The Authorization requirements do not apply, and a Security Accreditation Package is not required VA INFORMATION AND INFORMATION SYSTEM SECURITY/PRIVACY LANGUAGE FOR INCLUSION INTO CONTRACTS, AS APPROPRIATE ACCESS TO VA INFORMATION AND VA INFORMATION SYSTEMS All contractors, subcontractors, and third-party servicers and associates working with VA information are subject to the same investigative requirements as those of VA appointees or employees who have access to the same types of information. The level and process of background security investigations for contractors must be in accordance with VA Directive and Handbook 0710, Personnel Suitability and Security Program. The Office for Operations, Security, and Preparedness is responsible for these policies and procedures. The contractor or subcontractor must notify the Contracting Officer immediately when an employee working on a VA system or with access to VA information is reassigned or leaves the contractor or subcontractor s employ. The Contracting Officer must also be notified immediately by the contractor or subcontractor prior to an unfriendly termination. SECURITY INCIDENT INVESTIGATION The term security incident means an event that has, or could have, resulted in unauthorized access to, loss or damage to VA assets, or sensitive information, or an action that breaches VA security procedures. The contractor/subcontractor shall immediately notify the COTR and simultaneously, the designated ISO and Privacy Officer for the contract of any known or suspected security/privacy incidents, or any unauthorized disclosure of sensitive information, including that contained in system(s) to which the contractor/subcontractor has access. To the extent known by the contractor/subcontractor, the contractor/subcontractor s notice to VA shall identify the information involved, the circumstances surrounding the incident (including to whom, how, when, and where the VA information or assets were placed at risk or compromised), and any other information that the contractor/subcontractor considers relevant. With respect to unsecured protected health information, the business associate is deemed to have discovered a data breach when the business associate knew or should have known of a breach of such information. Upon discovery, the business associate must notify the covered entity of the breach. Notifications need to be made in accordance with the executed business associate agreement. In instances of theft or break-in or other criminal activity, the contractor/subcontractor must concurrently report the incident to the appropriate law enforcement entity (or entities) of jurisdiction, including the VA OIG and Security and Law Enforcement. The contractor, its employees, and its subcontractors and their employees shall cooperate with VA and any law enforcement authority responsible for the investigation and prosecution of any possible criminal law violation(s) associated with any incident. The contractor/subcontractor shall cooperate with VA in any civil litigation to recover VA information, obtain monetary or other compensation from a third party for damages arising from any incident or obtain injunctive relief against any third party arising from, or related to, the incident. LIQUIDATED DAMAGES FOR DATA BREACH Will not have access to the VA systems TRAINING No Training required, they will not be in patient areas or on the network. VA INFORMATION CUSTODIAL LANGUAGE The following standard Items relate to records generated in executing this contract: 1. Citations to pertinent laws, codes, and regulations such as 44 U.S.C. Chapter 21, 29, 31 and 33; Freedom of Information Act (5 U.S.C. 552); Privacy Act (5 U.S.C. 552a); 36 CFR Part 1222 and Part 1228. 2. Contractor shall treat all deliverables under the contract as the property of the U.S. Government for which the Government Agency shall have unlimited rights to use, dispose of, or disclose such data contained therein as it determines to be in the public interest. 3. Contractor shall not create or maintain any records that are not specifically tied to or authorized by the contract using Government IT equipment and/or Government records. 4. Contractor shall not retain, use, sell, or disseminate copies of any deliverable that contains information covered by the Privacy Act of 1974 or that which is generally protected by the Freedom of Information Act. 5. Contractor shall not create or maintain any records containing any Government Agency records that are not specifically tied to or authorized by the contract. 6. The Government Agency owns the rights to all data/records produced as part of this contract. 7. The Government Agency owns the rights to all electronic information (electronic data, electronic information systems, electronic databases, etc.) and all supporting documentation created as part of this contract. Contractor must deliver sufficient technical documentation with all data deliverables to permit the agency to use the data. 8. Contractor agrees to comply with Federal and Agency records management policies, including those policies associated with the safeguarding of records covered by the Privacy Act of 1974. These policies include the preservation of all records created or received regardless of format [paper, electronic, etc.] or mode of transmission [e-mail, fax, etc.] or state of completion [draft, final, etc.]. 9. No disposition of documents will be allowed without the prior written consent of the Contracting Officer. The Agency and its contractors are responsible for preventing the alienation or unauthorized destruction of records, including all forms of mutilation. Willful and unlawful destruction, damage or alienation of Federal records is subject to the fines and penalties imposed by 18 U.S.C. 2701. Records may not be removed from the legal custody of the Agency or destroyed without regard to the provisions of the agency records schedules. 10. Contractor is required to obtain the Contracting Officer's approval prior to engaging in any contractual relationship (sub-contractor) in support of this contract requiring the disclosure of information, documentary material and/or records generated under or relating to this contract. The Contractor (and any sub-contractor) is required to abide by Government and Agency guidance for protecting sensitive and proprietary information.
