(the Client) to conduct an asbestos and universal waste 1 /hazardous substance survey (survey) in advance of planned demolition of the former Oasis Teen Center addressed as 151 Hawley Street in Grayslake, Illinois (the Property). The scope of this survey was conducted by an Illinois Licensed asbestos building inspector/ Environmental Professional to inspect the materials of construction observed onsite for proposed demolition. This includes the identification of polychlorinated biphenyl containing components, mercury containing components, chemicals, and other materials that would require proper containment, storage, relocation, recycling, and/or removal and disposal. The survey was performed on April 26, 2023 by Mr. Mark Brumwell. The visual inspection of the Property identified the following suspect universal waste and hazardous substances: Asbestos-containing materials. Light bulbs/ballast (CFL, mercury vapor, fluorescent) (mercury, lead) Electrical transformer box, circuit breakers, and switch gear (Non-PCBs, petroleum) HVAC equipment (Refrigerants) Except for suspect asbestos, the above list of materials observed during the site reconnaissance were not sampled; therefore, they should be presumed to be universal waste/hazardous substances until confirmation samples are collected. Suspect universal waste/hazardous substances should be stored, containerized, recycled, temporarily relocated, removed, transported, and disposed in accordance with applicable federal, state, and local regulations prior to planned demolition and/or renovations. This survey was not intended to inventory routine maintenance chemicals used and stored at the Property.
