Site work for a civil project in Saint John, Indiana. Completed plans call for site work for a road / highway; and water / sewer project.
Any bids received after the above date and time shall be disqualified and will be returned to the prospective bidder unopened. The project includes the following Work: The public improvement is located in St. John, Indiana, and shall consist of water main replacement, sewer main installation, and roadway improvements to Acorn Drive, Acorn Court, and 96th Avenue. Water main improvements shall include the installation of 8-inch PVC water main, residential water services, hydrants, valves, and associated appurtenances. Sanitary sewer improvements shall include the installation of 8-inch PVC sanitary sewer main from the south end of Acorn Drive, under Bull Run, to the existing sewer in Alvina Rose Court, and the abandonment of the existing lift station. Roadway improvements will include HMA milling, HMA patching, and HMA paving as well as curb and gutter replacement and parkway restoration in areas where the existing roadway, curbing and parkway have been disturbed by the water main and sanitary sewer installations. The project will be let in compliance with Indiana Code (I.C.) Title 36-1-12 (Public Works Projects) and awarded to the lowest responsive and responsible bidder in the manner prescribed by the Indiana State Board of Accountants, and a Non-Collusion Affidavit, an EVerify Affidavit, an Iran Non-Investment Certification, and other documents as set forth on the Bid Form. The Issuing Office for the Bidding Documents is: Office of the Town of St. John Clerk-Treasurer 10955 W 93rd Avenue, St. John, Indiana 46373 Bid Addendum documents may be issued by the Project Engineer to Plan Holders registered with the Issuing Office. Prospective Bidders may obtain or examine the Bidding Documents at the Issuing Office on Monday through Friday during the Issuing Office's regular posted hours and may obtain copies of the Bidding Documents from the Issuing Office as described below. Partial sets of Bidding Documents will not be available from the Issuing Office. Neither the Owner nor the Engineer will be responsible for full or partial sets of Bidding Documents, including addenda, if any, obtained from sources other than the Issuing Office. Bids must be submitted on the Bid form included in these documents. The form must be fully completed and executed. No bids will be considered which are not based on these Contract Documents. Prequalification All Bidders must be properly licensed in the Town of St. John at the time bids are received. The Prime Contractor shall be INDOT prequalified for an amount greater than the value of the bid in any combination of Categories A(a), B(a), C(a), C(b), E(b), E(e), E(f), E(h), E(r), 0112 and 0188 and return any required prequalification information requested by the Town's Engineer during the bid process. When a subcontractor is utilized to complete work items, the subcontractor shall have respective INDOT prequalification for an amount greater than the value of the work items to be performed. Additionally, the Contractor shall have sufficient experience, as determined by the Owner and his representatives, in the field of municipal underground utility construction and paving to warrant release of the bid documents. The Contractor shall provide such documentation as is deemed necessary upon request. If this information is not satisfactorily completed, the bid documents shall be withheld. The Contractor shall have sufficient experience and shall demonstrate they have completed at least five (5) municipal underground utility and paving projects in Indiana or Illinois in the last five (5) years of which the utility work represented at least $700,000 of the contract. Upon the completion of the Work as defined by the Contract Documents, the Contractor (awarded Bidder) shall be required to furnish a maintenance bond, conditioned to guarantee the Work for a period of two (2) years.
