Attention of bidders is called to the federal requirements contained in this bid packet, including insurance requirements, equal opportunity provisions, and the requirement for a payment bond and performance bond for 100% of the contract price. The following forms must be completed and submitted as the bid package: Bid Guaranty & Contract Bond, Experience Statement, Time of Completion, Addenda Acknowledgment, Non-Collusion Affidavit, Property Tax Affidavit, Homeland Security Form, Guernsey County 25% Requirement, and Proposal Forms. Failure to complete the above forms and submit same at the bid opening will result in a non-responsive bid. Each proposal must be accompanied by a bond with an approved surety company as surety, in the sum of 100% of the amount of the bid as surety for the execution of the contract, or certified check for the amount of 10% of the bid on some solvent bank within the City of Cambridge, and made payable to the Director of Public Service, Cambridge, Ohio. Each proposal must contain the full name of the party or parties submitting the proposal and all persons interested therein.
