Project No. 552-23-512 REPAIR OPERABLE WINDOW UNITS Dayton VAMC The REPAIR OPERABLE WINDOW UNITS will be at the Dayton VA Medical Center located at 4100 W. Third Street, Dayton, OH 45428. DEFINITION AND ACRONYMS: Pre-Construction Risk Assessment (PCRA), Interim Life Safety Measures (ILSM), Request for Proposal (RFP), Certified Independent Third-Party Safety Professional (CITP) National Fire Protection Association (NFPA), The Joint Commission (TJC), Facilities Condition Assessment (FCA). Competent Person (CP), Contracting Officer s Representative (COR), Subject Matter Expert (SME). SCOPE OF WORK: General Requirements: Provide all supervision, labor, materials, tools, and equipment necessary for the construction of the Project described. All proposals shall include the following provisions that will be described in detail later in this document. Phasing and Infectious Controls. Privacy Training Testing of Representative Sample Health Screenings Sole-source Requirements Security & Badges VA Design Guides and TIL Facility Outage Request Form Project Overview: The existing operable windows in several buildings have severe leakage issues due to deteriorated gaskets and holes in the frame that allow water to enter the building. The intent of the project is to replace the gaskets on the operable windows of the South and West Elevations of Building 330 as detailed in Elevations 3 & 7 on sheet 103 and Elevations 11, 12 & 15 of sheet 104 on the original construction drawings that are provided with this solicitation. The windows were purchased by Wausau windows and a copy of the approved shop drawings and technical specs are included with this solicitation. Existing blinds shall be repaired, adjusted, and cleaned as needed for appearance and function. Protect windows from damage during removal, handling, and construction. Survey existing window units for additional damage when open and notify COR/ SME prior to performing any additional work. Clean all sides of glass panes while repairing units. Special attention is called to the fact that most of the windows are in occupied spaces, and many are in Veteran care rooms. The successful contractor shall include in their bid adequate consideration for phasing and will be required to develop a detailed sequence plan in conjunction with the COR/ SME. The floor plans from the original construction are included in the solicitation for informational purposes only. Bidders are responsible to verify the correctness of the plans during the pre-bid walk thru that will be scheduled. Shop Drawings for existing Wausau window units. Technical details for existing Wausau window units. SCHEDULING, PHASING, AND WORKING HOURS: All work hours MUST be coordinated with the project s COR. All work shall be performed during WHEN hours (Weekends, Holidays, Evenings and Nights) so as not to interfere with normal Hospital functions. However, some VA areas may be conducive to work being completed DURING normal business hours (7:00 am to 5:00 pm, Monday through Friday). All requests for work during normal business hours must be requested by the Contractor at least 14 calendar days in advance. Additionally, construction shall be performed (scheduled and phased) to ensure that exterior egress/ingress areas are completely operational by normal business hours the following workday. PROJECT DURATION Total Period of Performance for this Project is 180-calendar day duration. From Notice to Proceed, Contractor shall develop and submit a Critical Path Method (CPM) Schedule and Sequence Plan within 30-calendar days. Allow 14-calendar days for VA review and approval. From Notice to Proceed, Contractor shall provide submittal information within 45-calendar days. Allow 14-calendar days for VA review and approval. UTILITY OUTAGES Disruption of any facility system must be requested in writing by the Contractor utilizing the attached Outage Request Form through the COR at least 14-calendar days in advance. PROJECT CLASSIFICATION: This project has been classified per the Construction Infection Control Risk Assessment as a Type C, Medium Risk Group, Class III project. SAFETY REQUIREMENTS: All Contractor personnel are required to have completed an OSHA-approved 10-hour construction worker course prior to working on the site. The On-site general superintendent/supervisor is required to have completed an OSHA-approved 30-hour construction course prior to working on the site.A Every worker must provide the Contracting Officer a copy of current certificates identifying successful completion of the training prior to executing work for the Veterans Administration. In addition, Contractor shall provide submittals for contract construction or renovation work which include the names, qualifications, and training dates for Contractor s Competent Persons (CP), who has been designated to administer the site-specific safety program, as well as the CP for other activities as required by OSHA regulation 29 CFR 1926.32 (such as scaffolds, cranes, excavations, etc.).A Contractor s CP are required to implement and maintain effective safety programs that identify and control hazards that may cause injury or illness to VA patients, staff, visitors, and contractor employees.A The VA CP does not take the place of Contractor s CP nor acts on their behalf. A The VA CP determines if Contractor is meeting VA standards and contractual requirements for safety and OSHA compliance. A When these standards and contract requirements are not being met, the VA COR and/or CP will take immediate action to prevent injury, non-compliance, and/or property damage. The Contractor shall submit an Activity Hazard Analysis, and an Accident Prevention Plan as per FAR 52.236-13 prior to commencement of site activities.A The Government shall have 21 calendar days to review the AHA and APP.A PRIVACY REQUIREMENTS: All DB employees and subcontractor employees, requiring access to VA information, shall complete the following before being granted access: Successfully complete the VA Privacy and Information Security Awareness Course and Rules of Behavior training, and annually complete required privacy/security training. Successfully complete any additional cyber security or privacy training, as required for VA personnel with equivalent information access. The contractor shall provide to the VA COR a copy of the training certificates for each applicable employee, within 1 week of the initiation of the contract and annually thereafter, as required. Print the training certificate at the end of the session or; Review the text version of the training modules and complete the certificates found at the end of the training modules. GOVERNMENT FURNISHED INFORMATION: All available record drawings and documents that have been provided shall not be considered as-built documents. These drawing files are provided without warranty or obligation as to the accuracy or information contained in the files. All information in the files shall be independently verified by the user. Any user shall agree to indemnify and hold the VA harmless from any and all claims, damages, losses, and expenses, including, but not limited to, attorney fees arising out of the use of the Computer Aided Design and Drafting (CADD) drawing files. Test shall be witnessed and verified by the COR and SME. Test report shall be prepared per the guidance in the Standard and be signed by all parties involved in the test. AIR INFILTRATION: Contractor shall test six (6) windows that serve as a representative sample of the replacement windows for air infiltration in accordance with ASTM E331. At the conclusion of the tests, there shall be no glass breakage or damage to fasteners, hardware parts, actuating mechanisms or any other items rending windows inoperable. Submit copies of test reports from an Independent Testing Laboratory and certificates signed by the window supplier or manufacturer stating that the windows delivered to the site comply with all specified performance requirements. All employees of general contractor and subcontractors shall comply with VA security management program and obtain permission of the VA police, be identified by project and employer, and restricted from unauthorized access. SET-ASIDE INFORMATION: This solicitation will be issued pursuant to the authority under Public Law 109-461 (38 U.S.C. 8127) and is a 100% Set-Aside for Service-Disabled Veteran-Owned Small Business (SDVOSB). VETCERT & SAM: in accordance with VAAR 819.7003(b) (Deviation), at the time of offer/quotes (SF330 submission), and at the time of award of any contract, the offeror must represent to the contracting officer that it is a (1) SDVOSB eligible firm; (2) Small business concern under the North American Industry Classification System (NAICS) code assigned to the acquisition; and (3) Deviation [Certified] SDVOSB/VOSB [listed in] the [SBA certification database (aka VetCert) at https://veterans.certify.sba.gov/ (see 13 CFR 128)] If registration is required, visit https://veterans.certify.sba.gov/. The project NAICS code is 236220 and the business size standard is $39.5 million. In accordance with Veterans Affairs Acquisition Regulation (VAAR) 852.219-10(c) (4), the SDVOSB is required to perform a minimum of 15% of the construction work on the project. BONDS in form of Bid, Payment, and Performance will be required. SITE VISIT: a one-time only site visit will be scheduled. The Site Visit information will be in the solicitation package. The solicitation documents will be posted at www.fbo.gov on or about July 23, 2023, with bid opening approximately 30 days later. Dissemination of the solicitation, specifications and drawings is limited to electronic medium.
