SECTION 1 INTRODUCTION & PROJECT SUMMARY This Request for Proposal (RFP) for construction-related supplies and services to install a footer and slab on the North side AWOL at 116 S. 23rd Street, Independence, Kansas. Material and labor supplied by Contractor: o 18' x 45' slab extension to the existing structure at 116 S. 23rd Street. o Pour and finish concrete footer and slab on the north side of AWOL. The floor slab will be to a smooth finished floor quality. Reference the attached engineer design. o Cement Curing compound applied to the slab surface after pouring. o Form, pour, and smooth finish slab as requested by the RFP. o All work ready for pre-pour inspection by the City Building inspector. Material supplied by Owner: o City will provide the building permit. o Inspection cost SECTION 2 SCOPE OF WORK PHASE 1 - Estimate of work. The City of Independence requests a proposal to install a slab at AWOL. The contractor shall provide and pay for all materials, labor, tools, equipment, removal, and disposal of debris material, clean site, and All supplies and materials shall be new. PHASE 2 - City will award the contract.
