The Community Health Centers of Western Kentucky will receive lump sum sealed bids on a general contract for construction of a new doctor's office building. The Project name is "COMMUNITY HEALTH CENTERS OF WESTERN KENTUCKY NEW OFFICE BUILDING, GREENVILLE, KENTUCKY", as shown on Contract Documents prepared by the Architect, RBS Design Group, P.S.C. Drawings and specifications are dated July 10, 2023 Abbreviated Written Summary: Briefly and without force and effect upon the contract documents, the work of the Contract can be summarized as follows: Project Description: Construction of a single-story medical office building of approximately 8,000 square feet with a mechanical mezzanine. The Work includes, but is not limited to site clearing, earthwork, termite control, hot mix asphalt paving, chain-link fences and gates, lawns and grasses, cast-in-place concrete, simulated stone masonry, unit masonry assemblies, structural steel, metal fabrications, rough carpentry, wood roof decking, metal plate connected wood trusses, gluedlaminated construction, interior architectural woodwork, building insulation, asphalt shingles, siding, soffits, sheet metal flashing and trim, firestopping, joint sealants, standard steel doors and frames, flush wood doors, aluminum-framed entrances and storefronts, wood windows, door hardware, glazing, gypsum board assemblies, acoustical panel ceilings, resilient wall base and accessories, carpet tile, acoustic wall panels, painting, toilet compartments, signs, post and panel signs. fire protection specialties, protective covers, toilet and bath accessories, HVAC & plumbing systems, electrical (power & lighting) systems, and clean-up and removal of construction debris. Proposals will be received as a Lump Sum Bid(s) for the work indicated in the Bidding Documents. Checks covering deposit shall be made payable to the Owner. All mailing charges (non-refundable), if required, are to be arranged in advance with and paid directly to Maco-Evansville Blue with a separate check. Electronic copies of the documents are available from Maco-Evansville Blue, 600 Court Street, Evansville, IN. (812-464-8108). Charges to obtain electronic copies are non-refundable and are to be arranged with, and paid directly to, Maco-Evansville Blue. Deposits will be refunded to contractors who submit a bona fide proposal to the Owner (directly or through a prime contractor) and return the documents, in an acceptable and reusable condition within ten days of the opening of the bids. Partial sets of documents are not available from the office of the Architect. Bid Guaranty in the form of a Bid Bond (Proposal bonds with Contractor acting as surety are not acceptable), Certified Check, or cash shall accompany each bid. The amount of the bid Guaranty shall be not less than five percent (5%) of the Bid set forth in the Proposal Form. All bid Guaranties shall be made payable to the Owner. No bids may be withdrawn after the scheduled closing time for receipt of bids for a period of sixty (60) days therefrom. The Owner reserves the right to determine the ability of any bidder to perform the work, and upon the request of the Owner, any bidder shall furnish such information as may be required to make this determination. The successful bidder shall be required to furnish a performance and payment bond and a material and payment bond in an amount equal to at least One Hundred Percent (100%) of the Contract price. The premium for the bonds shall be included in the proposal amount(s). The Owner reserves the right to accept any bid, to reject any or all bids, to waive any informalities in bids received where such acceptance, rejection or waiver is considered to be in the best interest of the Owner and to reject any bid where evidence of information submitted by the bidder does not satisfy the Owner that the Bidder is qualified to carry out the details of the Contract Documents. The Substantial Completion date shall be midnight, July 1, 2024.
