PMH is requesting proposals from qualified contractors to complete audit/preventative maintenance work on various pitched and flat roof systems through out the region. Scope of Work Included in Proposal: 1. Complete an assessment of each facility roof 2. Detailed report identifying major/minor issues for each facility including photos and budgetary numbers of what is required. 3. Reports shall include a five-year forecast of repairs/replacement 4. Repair minor issues and clean debris from around roof drains, scuppers and eavestrough to a maximum amount set by PMH 5. A sample of a completed roof report must be added to proposal
