Description PARKER TRAINING/AIRPORT TERMINAL RESTROOM REMODEL The City of Findlay has recently completed renovation projects at the Parker Training Facility located at 315 South West Street (entrance off West Crawford Street) and the Airport Terminal located at 1205 Lima Avenue. These projects will complete the renovation projects by adding restrooms and removing the existing at the Parker Center and by renovating the restrooms at the Airport Terminal. Work will include all necessary ADA provisions. Engineering Estimate: $ 430,000 Airport Terminal: Substantial Completion Date: March 15, 2024 Final Completion Date: March 29, 2024 Parker Training Facility: Substantial Completion Date: April 19, 2024 Final Completion Date: May 3, 2024 Engineering Estimate: Parker Training Center Restrooms: $260,000 Airport Terminal Restrooms: $150,000 Bond Percentage 10.00% Keep full and accurate payroll records available for inspection for up to one (1) year following the completion of the project. Owners/partners must be shown on payrolls for any physical work they perform on the job. Supply Findlay Prevailing Wage Coordinator with dates of the life of the contract and a listing of all subcontractors to include the name, address, and telephone number of each. Supply each employee with written notification of his assigned job classification, hourly prevailing wage rate and fringe payments, and identity of the Prevailing Wage Coordinator for the project. Supply all subcontractors with any changes in the Prevailing Wage rates issued during the life of the project. Out-of-State contractors must submit to the Ohio Secretary of State, the full name and address of their Statutory Agent in Ohio. Submit certified payrolls starting no later two (2) weeks after the initial pay period. Insure all payrolls include the following: A. Employees full name, address, and social security number B. Work classification (1) Be specific when listing laborers and operators (2) Show level/year for all apprentices C. Hours worked on the project (1) As of May 1, 1984, overtime is to be paid at not less than time and one-half for all hours worked in excess of forty (40) hours per week. D. Hourly rate of pay Actual hourly rate paid employee for the time worked. Overtime hourly rate of not less than time and one-half the basic or regular rate paid is required under Chapter 4115. In addition to paying not less than the predetermined rate for the classification is which the employee works, the amounts predetermined as fringe benefits in the wage determination issued for the project, shall also be paid. (Sample calculation: $12.82 (base rate) x 1 1/2 = $19.25 + $1.70 (fringe payments) = $20.93). Fringe payments must be entered in appropriate blocks on payroll forms when such fringes are paid to approved plans, funds, etc. E.
