This solicitation is for the repair, blasting and painting of the water tank for the Louisiana Department of Health (LDH), Pinecrest Support and Services Center located at 100 Pinecrest Drive, Pineville, Louisiana 71360. The job is to be performed in accordance with the enclosed scope of work and specifications. Please see attachments for information about insurance, builder's risk, bond, specifications, jobsite visit, and contractor's license requirements. Please print all attachments to ensure all documents related to this solicitation are reviewed prior to bidding. All documents associated with this solicitation should be included in bidder's submission. In accordance with La. R.S. 37:2165 A (1), a Contractors' License Number in the appropriate classification(s) such as Building Construction and/or 7-48 Municipal and Public Works, Building Construction and/or 7-48 Painting, Coating and Blasting must appear on the bid opening envelope on all projects in the amount of $50,000.00 or more (and $1.00 or more if hazardous materials are involved). Failure to do so shall remove the bid from consideration. In accordance with La. R.S. 37:2165 C, anyone objecting to the above classification(s) must send a certified letter to both the Louisiana State Licensing Board for Contractors and the Office of State Procurement. The letter must be received no later than ten (10) working days prior to the day on which the bid is to be opened. For any bid submitted in the amount of $50,000 or more, a bid security (insurance company, bank money order, certified check, or cashier's check) must be included with the bidder's submission in the sum of five percent (5%) of the amount bid (including base bid and additive alternates, if any) and shall become the property of the agency in the event the contract and bond are not executed within the time set forth in the bid solicitation. Failure to do so will remove the bid from consideration.
