The Commonwealth of Pennsylvania, Department of Corrections, requires an Ascom nurse call system at State Correctional Institution - Albion, 10745 PA-18, Albion, PA 16475. This solicitation is for the furnishing & installing of a Ascom Nurse Call System per attached statement of work for the Commonwealth of Pennsylvania, Department of Corrections. See attached specifications for details of requested materials. No Substitutions. All bids must be submitted electronically. All items will be delivered FOB Destination. The awarded supplier is responsible for any shipping or delivery costs. Requested delivery date is estimated and a mutually agreed upon delivery will be determined at the time of award. Contractor's electronic submission must include the following as attachments. Failure to submit these items may result in rejection of bid. After the bid opening, and prior to awarding the contract, the Department has the right to request references (name, addresses and telephone numbers) of similar work performed in the previous two (2) years as proof of qualifications to perform the work involved in this contract. 1. Completed BOP-2201: Worker Protection and Investment Form Pursuant to Executive Order 2021-06, Worker Protection and Investment (October 21, 2021), the Commonwealth is responsible for ensuring that every Pennsylvania worker has a safe and healthy work environment and the protections afforded them through labor laws. To that end, contractors and grantees of the Commonwealth must certify that they are in compliance with all applicable Pennsylvania state labor and workforce safety laws. Such certification shall be made through the Worker Protection and Investment Certification Form (BOP-2201) and submitted with the bid, proposal or quote. DOC will award a contract to the lowest responsive and responsible contractor. The term of the Contract shall commence on the Effective Date (as defined below) and shall end on the Expiration Date identified on the Contract, subject to other provisions of the Contract. The Effective Date shall be: a) the Effective Date printed on the Contract after the Contract has been fully executed by the Contractor and the Commonwealth (signed and approved as required by Commonwealth contracting procedures) or b) the "Valid from" date printed on the contract, whichever is later.
