Jefferson Patterson Park & Museum (JPPM) is seeking a firm to replace the existing Walk-In Freezer & Cooler, including demolition, removal, and disposal of the current units, as outlined in the attached Project Specifications entitled "MAC Lab Walk-In Cooler & Freezer Replacement" and Invitation for Bid form. The proposal package must be submitted according to Section III, "Bid Submission Packet", and must include: 1. Technical Proposal based on specifications outlines in the Invitation for Bid (IFB.) 2. Financial Proposal in separate document from Technical Proposal. 3. Completed "Bid/Proposal Affidavit". 4. Completed "Conflict of Interest and Disclosure Affidavit". 5. Completed "Mercury Affidavit" 6. Completed "Contract Affidavit" 7. Completed "Small Business Contract Affidavit" (if applicable.) 8. Completed "Vendor Company Profile" 9. References, submitted on the Reference Form. 10.Current insurance certificates. 11.Acknowledgement of receipt of any amendment(s) to this IFB. Please supply a letter on the company's letterhead acknowledging receipt of amendments. 12.Completed No Reply Form if not bidding. MBE (0%)
