The Proposed Work Consists of but is Not Limited to; Furnish and Installation of New Column Piping, Line Shaft Assembly and Bowl Assembly, Servicing of Motor, Removal & Reinstallation of Discharge Head Assembly, Well Video Inspection & Taping, Specific Capacity Testing, Well Redevelopment, Disinfection, and Any Related Site Restoration Work. The Proposed Work is Located; at the Willliam J. Miller Water Treatment Plant at 1551, Highway 888 West, Brick, Ocean County, New Jersey Note: This Project is Not Funded by the New Jersey Environmental Infrastructure Financing Program (Njeifp). Each Bid Must Be Accompanied by a Bid Bond or Certified Check for an Amount Equal to Ten Percent (10%) of the Bid or a Maximum of $20,000.00; Such Certification to Be Without Condition or Qualification, and the Check Made Payable to the Brick Township Municipal Utilities Authority. Each Bid Must Also Be Accompanied by a Consent of Surety, Properly Executed by a Surety Company Authorized to Do Business in the State of New Jersey or Permitted Individual And Shall Be Attached to the Proposal. All Bidders Shall Be Required to Comply With the Requirements of N.j.s.a. 10:5-31 Et Seq. And N.j.a.c. 17:27-6.2, Requiring Each Bidder and Each Subcontractor to Formulate and Submit to the Proper Authorized Representative of the State the New Jersey Department of Labor and Workforce Development an Affirmative Action Program of Equal Opportunity Which Guarantees Minorities Employment in All Employment Categories. The Bidder is a Corporation, a Partnership or a Limited Liability Company, the Bidder, in Conformance With N.j.s.a. 52:25-24.2, Shall Submit With His Bid a Statement Setting Forth the Names and Addresses of All Stockholders in the Corporation Who Own 10 Percent or More of Its Stock, of Any Class, or of All Individual Partners in the Partnership Who Own a 10 Percent or Greater Interest Therein, or of All Members in the Limited Liability Company Who Own a 10 Percent or Greater Interest Therein, as the Case May Be. If One or More Such Stockholder or Partner or Member is Itself a Corporation or Partnership or Limited Liability Company, the Stockholders Holding 10 Percent or More of That Corporation's Stock, or the Individual Partners Owning 10 Percent or Greater Interest in That Partnership, or the Members Owning 10 Percent or Greater Interest in That Limited Liability Company, as the Case May Be, Shall Also Be Listed.
