Site work and paving for a mixed-use development in Middleburg, Florida. Completed plans call for site work for a road / highway; for paving for a road / highway; sidewalk / parking lot; sidewalk / parking lot; and water / sewer project.

Last Date of Inquiries:December 22, 2023, 4:00pm Final Addendum:January 3, 2024, 4:30pm Purpose Clay County requests bids to perform the construction of roadway and drainage improvements along County Road CR 218 from Cosmos Avenue to Pine Tree Lane in Middleburg, Florida. The project has five (5) separate components: CR 218 Base Bid, CR 218 CCUA Bid, CR 218 Optional Bid 1, CR 218 Optional Bid 2, and CR 218 Optional Bid 3. 3.2. Scope A. CR 218 Base Bid: Roadway reconstruction from Pine Tree Lane to Masters Road. The existing two-lane rural roadway will be reconstructed to create a four-lane urban section with bike lanes and sidewalks for approximately 1.3 miles. The Design Speed is 45 mph. The raised median will vary between 14 ft. and 16 ft. wide and include Type E curb and gutter. Travel lanes will be 11ft wide (Inside & Outside). Buffered bike lanes will be 5 ft. wide and located along both directions of travel. Border areas will include Type F curb and gutter and a 5 foot (minimum) concrete sidewalk. The proposed typical section described above is the minimum requirement for the Project. The Contractor will be required to install median opening locations with associated turn lanes as depicted in the construction plans. Four (4) stormwater facilities will be constructed per the permit. B. CR 218 CCUA Bid: The Contractor will be responsible for coordinating utility relocations per the Utility Work Schedules. The Contractor shall be responsible for determining, through the use of non-destructive means, both the horizontal and vertical location of all existing utilities above and below ground within the project limits, and for coordinating with the Utility owner (s) for any necessary relocation and/or adjustment of their utilities per the utility work schedule. The Utility Work Schedules will be provided with RFB Amendment No.1. The Contractor is required to perform the utility relocation work for Clay County Utility Authority. The work effort for these planned relocations is described below and included as a component set to the construction plans. 1. Clay County Utility Authority (CCUA) The Contractor shall be required to construct and prepare as-built plans for the CCUA water and sewer adjustment/relocation work as approved by CCUA and the County. All water and sewer construction, tie-ins to existing water and sewer system, service connections and resolution of conflicts with existing water and sewer systems shall be in accordance with the current CCUA standards and shall be scheduled with and coordinated through CCUA. The Contractor shall copy the County on all correspondence with CCUA related to schedule and coordination efforts. This work is subject to the contract agreement between CCUA and the County and may be removed from the contract at CCUA's discretion. Utility Work Construction costs for mobilization, clearing and grubbing and maintenance of traffic for this utility work are to be included in the CR 218 Base Bid Price and not in the utility relocations cost. The utility work will include all utility facilities of CCUA which are located within the limits of the Project. The Contractor shall fully cooperate and coordinate the utility work with all other right-of-way users. The Contractor shall facilitate and comply with all permit conditions, and provide all disinfection, pressure testing, laboratory tests, permit certifications, record drawings, etc. to obtain regulatory approval and clearance to place the utilities in service. Performance of Utility Work The County shall perform construction engineering inspection and monitoring of the Utility Work for compliance with the Contract Documents. The CCUA shall have representation on site periodically for consultation as necessary. Testing, monitoring and reporting of the Utility Work shall be performed by the Contractor in accordance with standard industry practices and permit conditions for water and wastewater and in accordance with the CCUA's standards. The Contractor shall coordinate with the CCUA during construction and schedule any necessary temporary utility interruptions, in advance with the County and CCUA. All out of service CCUA mains, services, and appurtenances that are in conflict with the construction plans shall be removed. Should out of service facilities not require removal, then Contractor shall cap them or grout fill and place them out of service in accordance with FDOT Specifications 430-4.5. All work described above for the CCUA utility relocations shall be bid under the separate line item for CCUA and included in the Bid Form. The Contractor shall minimize and, to the greatest extent possible, avoid impacts to existing landscaping. C. CR 218 Optional Bid 1: Intersection Improvements at Mimosa Ave. The existing two-lane rural roadway approaches and intersection will be reconstructed to create a four-lane urban section with bike lanes and sidewalks for approximately 0.25 miles. Turn lanes with traffic separators will be constructed in each direction. Travel lanes will be 11ft wide (Inside & Outside). Buffered bike lanes will be 5 ft. wide and located in both directions of travel. Border areas will include Type F curb and gutter and a 5 foot (minimum) concrete sidewalk. The proposed typical section described above is the minimum requirement for the Project. Traffic signal modification/installation/removal, including strain pole construction is a requirement of this Bid Option. The existing signalized intersection will require replacement to accommodate the new four-lane section. Replace existing signal structures with strain poles at the intersections of CR 218 and Mimosa Avenue per the construction plans. The signal design includes full pedestrian features with countdown pedestrian signals meeting ADA standards, LED signal heads with reflective back plates, vehicular detection (video), LED internally illuminated overhead street name signs (servicing all applicable directions), Uninterrupted Power Supply devices (UPS), and emergency preemption devices. Lighting within the limits of the signalized intersections will be improved with enhanced horizontal and vertical lighting illumination standards as per the construction plans. D. CR 218 Optional Bid 2: Sidewalk from Mimosa Ave to Cosmos Ave A concrete sidewalk will be constructed on the south side of CR 218 from Mimosa Avenue to Cosmos Avenue for approximately 0.65 miles. The sidewalk will be 6 feet wide by 4 inches thick and have a 1-foot minimum separation from the edge of the existing pavement. Additional work includes grading the back of sidewalk to existing grade, sodding of disturbed areas and driveway reconstruction. E. CR 218 Optional Bid 3: Engineer's Field Office The Contractor will provide an Engineering Field Office in accordance with Section 109 of the attachments (minimum 900 sf). The field office shall be a building or mobile trailer erected at locations selected by the Contractor as approved by the County. Requirements: This project includes all associated work shown in the plans and specifications attached to this bid. All construction shall comply with the Florida Department of Transportation (FDOT), FY2023/2024 Standard Plans for Road and Bridge Construction including Interim revision and FDOT's July 2023 Standard Specifications for Road and Bridge Construction. Contractors must abide by all documented requirements and general notes as listed in the construction plans. Contractors are required to submit FDOT Pre-Qualified Fields listing provided for in attachments. The Awarded Contractor shall provide a Critical Path Method (CPM) production schedule meeting the contract days. This schedule shall be held to close scrutiny due to the impact to inspection time. If the Contractor does not hold to the production schedule, the Contractor may be required to pay for inspection services for downtime of production. All work shall be contained within the County's right-of-way. No work may be performed outside of the County's existing right-of-way unless specifically approved by the County. The Contractor shall provide and furnish all construction activities, utility work, tools, equipment, supervision, labor, materials, rentals, subcontractors, profit, overhead, other costs, and services as described in the RFB related to this construction project. The Contractor will be required to provide proper access to businesses and side streets to ensure the safety and efficiency of the transportation system is maintained. This includes but is not limited to providing reconstructed driveways at all locations where driveways currently exist. All driveways and side roads shall be reconstructed or resurfaced to the County's right-of-way line unless otherwise noted in the construction plans. All driveways and side roads shall remain open at all times. The Contractor shall construct sidewalks for the full limits depicted in the construction plans. The Contractor will be required to comply with all right-of-way, project and environmental commitments (and/or agreements) attached to this Bid. The Contractor shall provide a fully functional drainage system to accommodate the requirements of this project. This includes, but is not limited to, replacement of existing structures and other drainage improvements necessary to provide a fully functional four-lane facility along CR 218 to meet water quality and quantity permitting requirements. All existing drainage components within the project limits shall be removed or grout-filled and annotated on as-builts. Capping of existing pipes without grout filling will NOT be accepted. State and Federal regulatory permits are being acquired through the SJRWMD and the USACE and modifications to these permits are not anticipated. Any changes to the permitted design will require County approval. Any modifications to one or both permits will be the sole responsibility of the Contractor, including applicable application fees and noticing costs. Based on the submitted construction plans, there are unavoidable impacts to wetlands and compensatory mitigation credits have been acquired and accepted by both agencies. Any additional wetland mitigation required due to design changes is the responsibility of the Contractor. Irrigation that encroaches into the right of way shall be cut and capped and the property owner shall be notified prior to cutting and capping. The adjacent property owner shall also be notified if any landscaping within the County's right of way is removed. The Contractor shall provide the adjacent property owner a minimum one-month advanced written notification to allow the property owner time to remove the irrigation or landscaping from the County's right of way. After that one-month notification period ends, the Contractor will cut/cap irrigation, remove remaining irrigation components and any impacted landscaping from the County's right-of-way. It is the intent to always preserve existing vegetation including trees and palms that do not conflict with the proposed improvements. Tree and palm protection shall comply with FDOT Standard Plans for Road and Bridge Construction (Standard Plans), Index 110-100. Within the Project limits and within the Project Right of Way, it will be the responsibility of the Contractor to identify and remove all Category 1 invasive exotics as defined by the Florida Exotic Pest Plant Council (www.fleppc.org). The intent of this Project is to replace, repair or rehabilitate all deficiencies noted in the construction documents within the Project limits such that maintenance work required upon Final Acceptance is limited to routine work. All construction activities for the project will be required to remain within the existing right-of-way. An old set of Right-of-Way Maps is made available in the reference documents. During the right-of-way acquisition process, the County obtained rights-of-entry or easements from property owners and documented this specific access right in the Right-of-Way Commitment Plans. For this reason, the Right-of-Way Commitments that include property access rights shall overrule the Right-of-Way Maps and the CDs. The Contractor shall construct all Right-of-Way Commitments and elements included in the RW Commitment plans. The Contractor will be responsible for clearing and grubbing including, but not limited to existing fencing, trees, concrete removal, etc. Additional activities include but are not limited to the following: signalization, maintenance of traffic, excavation and grading, erosion and sediment control, sidewalk, asphalt paving, drainage, handrail, fencing, sodding, signs and pavement markings. 3.3. Contractor and Subcontractor Qualifications 1. The Contractor shall be licensed to perform all work listed in the Scope of Work provided. 2. Contractor shall obtain all permits to complete the project in accordance with the contract documents. 3. All work will be performed in accordance with the most applicable state and local regulations. 4. The Contractor shall own or have full access to the appropriate personnel and equipment to complete the project requested. 5. The County reserves the right to approve all subcontractors for this contract. If subcontractors are to be utilized, the subcontractors must possess an active and valid license issued by the Florida Department of Business and Professional Regulation in the appropriate category. Sub-Contractors may be added during the contract period only with PRIOR WRITTEN PERMISSION from the County, and only for reasonable cause, as judged by the County. Responsibility for the performance of the contract remains with the main contractor exclusively. 6. The Contractor and Sub-Contractors may be requested to provide a list of equipment with identification (i.e. serial #, VIN, etc.), personnel with their position (e.g. foreman, laborer, etc.), and percentage of work performed. 7. Contractors must have been in business for a minimum of 5 years doing similar work. LAST DATE FOR INQUIRIES: January 10, 2024, 4:00 pm.
