Notice Content Request for Bid The Hazlehurst City School District (Hcsd) is Soliciting a Request for Bid From Qualified Contractors to Provide Services for Painting Services Per the Attached Scope of Work and Location and System Designated Within the Specifications. Questions Regarding This Solicitation Should Be Directed to Mr. Cloyd Garth, Jr. Superintendent of Education, by Email at Cgarth@hazlehurst.k12.ms.us or by Phone at (601) 894-1152. For Specific Instruction on Information to Include in the Quotation Refer to the Section Entitled Quotation Elements. Evaluation of the Proposals and Ultimate Selection is Identified in the Section Entitled Selection Process and Evaluation Criteria. Project Description Hcsd is Committed to Continue in Its Work to Keep the Districts Facilities Updated and Operating in an Efficient Manner. The Proposed Services Will Require Contractors to Execute the Project and to Assure That All Work is Conducted According to the Laws Regulations Codes That Govern Our State Department of Education, Building Codes, Mechanical Codes and the Mississippi Contractors Board the Project is Summarized as Follows: Scope of Work The Scope of Work Will Include, at a Minimum, the Following Elements: 1. Painting Contractor Shall Apply Two (2) Coats of Paint to the Following Surfaces: Walls, Door Frames, and Ceiling Grid System. 2. Contractor Shall Be Responsible for Contacting Service Providers. 3. Site Visit to Verify the Scope of Work and Conditions of Existing Site. 4. Provide Skilled Craftsmen to Perform Work and Process Required to Replace Designated Equipment for Replacement. 5. Adjustments Can and Shall Be Made by the Owners Representative as Required and Approved by I He Owner by Way of Written Change Orders and/or Change Directives. 6. Contractor Shall Clean and Prepare Site After Work is Completed. Quotation Elements 1. Scope of Work: Contractor Shall Provide All Labor, Equipment, Supplies, Taxes, Insurance, Fuels, Permits, Retirement of Utilities, and Any and All Other Items Necessary to Complete the Work and Disposal of Materials and Related Work, as Specified Herein. Contractor Shall Complete All Work (Demolition, Installation, and Site Cleanup). 2. Investigation of Conditions: Before Submitting a Bid, Bidders Should Carefully Examine the Specifications and Visit the Site to Fully Understand the Scope of Work. 3. Location of Project: 112 School Drive., Hazlehurst, Ms 39083 General Requirements: 1. Survey of Contents: the Contractor Shall Apply Paint to Walls, Doorframes, and Ceiling Grids. Owner Will Provide Painting Materials (Paint, Caulking, Etc.) Contractor Shall Be Required to Verify Quantities Prior to Submittal of Bid. 2. Demolition and Removal: Demolition and Removal Shall Include, as Applicable: a. Fences Shall Remain, Unless Otherwise Instructed to Remove Them. B. Clean-up: All Demolition Materials and Debris Shall Be Removed From the Work Concurrently With Progress of Work. Contractor Shall Not Allow Mud and Debris From Vehicle Transporting Demolition Materials to Litter Surrounding Area. 3. Insurance Requirements: a. Liability Insurance Shall Include All Major Divisions of Coverage and Be on a Comprehensive Basis Including: 1. Premises -operations (Including X-c-u) 2. Independent Contractors Protective 3. Product and Completed Operations 4. Contractual - Including Specified Provisions for All Contractors Obligations 5. Owned, Non-owned, and Hired Vehicles 6. Broad Form Coverage for Property Damage B. Limits: the Insurance Required Shall Be Written for Not Less Than the Following, or Greater is Required by Law: 1. Workmens Compensation- Statutory Coverage 2. Employers Liability- $100,000 3. Comprehensive General Liability 4. Bodily Lnjury - $500,000 Each Person a. $500,000 Each Occurrence 5. Personal Injury- $500,000 Each Person a. $500,000 Aggregate 6. Property Damage-$500,000 Each Person a. $500,000 Annual Aggregate 7. Projects Over $500,000- $1,000,000 Umbrella 8. Comprehensive Automobile Liability 9. Bodily Lnjury - $250,000 Each Person a. $500,000 Each Occurrence B. $500,000 Combined Single Unit 10. Property Damage-$100,000 Each Occurrence a. $500,000 Combined Single Unit C. Furnish Certificate of Insurance Specifically Setting Forth Evidence of All Coverage Required. Form of Certificate Shall Be Ala Document G705 or Similar Company Form. Furnish Owner Copies of Any Endorsements Subsequently Issued Amending Coverage Limits. D. Performance and Payment Bond as Required by the State of Mississippi Department of Education Purchasing and Construction Specifications 4. Owners Liability Insurance: a. Contractor Shall Furnish Insurance to Protect Owner From His Contingent Liability to Others for Damages Because of Bodily Injury, Including Death, Which May Arise From Operations Under This Contract and Other Liability for Damages, Which the Contractor is Required to Insure Under Any Provision of This Contract. Certificate of This Insurance Shall Be Filed With Owner With the Same Limits as Set Forth Above. 5. Property Damage: a. Contractor Shall Maintain Property Insurance Upon the Entire Work at the Site. Coverage is Limited to Work Under This Contract and the Contract Amount. B. Insurance Shall Include the Interests of the Owner, Contractor, Subcontractors, and Material Suppliers in the Work and Shall Insure Against Perils of Fire and Extended Coverage and Shall Include All Risk Insurance for Physical Loss or Damage Including Theft, Vandalism, and Malicious Mashie. C. Contractor Shall Furnish Copies of All Policies Before Exposure to Loss Can Occur. 6. Specifications: a. All Repairs Shall Be Per Specification Sheet Brand or Equal. B. All Work Shall Be Accomplished in Accordance With This Scope of Work and the Specifications Contained or Referenced Herein and in Accordance With All Local, State, . And Federal Rules, Laws, and Regulations. Selection Process and Evaluation Criteria: a Committee of the Districts Personnel Will Evaluate and Rate of the Proposals. Following Evaluation of the Quotations, Hcsd Will Interview Prospective Contractors. Those Firms Selected for Interview Will Have the Opportunity to Present Their Past Experience in the Heating Ventilation Air Conditioning (Hvac) Field and Overall Project Approach. Each Contractor Shall Be Notified in Writing of the Results of the Selection Process. Terms and Conditions: Hcsd Reserves the Right to Reject Any and All Bids and to Waive Irregularities and Informalities in the Submittal and Evaluation Process. This Solicitation for Construction Services Does Not Obligate Hcsd to Pay Any Costs Incurred by Respondents in the Preparation and Submission of a Bid. This Solicitation Does Not Obligate Hcso to Accept or Contract for Any Expressed or Implied Services. Furthermore, Hcsd Reserves the Right to Award the Contract to the Next Most Qualified Contractor if the Selected Contractor Does Not Execute a Contract Within Thirty (30) Days After the Award of the Bid.
