Contractor agrees to provide to the Department of California Highway Patrol (CHP) Central Los Angeles Area Office, the services described herein: furnish all supplies, materials, tools, equipment, labor, personnel, and supervision; pay all taxes, insurance, bonds, license and permit fees, and all other direct and indirect costs necessary to provide Flooring Services in accordance with the terms set forth in this Agreement. The services shall be performed at: Department of California Highway Patrol Central Los Angeles Area office 777 West Washington Boulevard Los Angeles, CA 90015 Contractor shall forfeit to the State as liquidated damages, One Hundred Dollars ($100.00) for each calendar day, or option thereof, which is delayed beyond the established work schedule, to be deducted from any payments due or to become due to the Contractor. Contractor shall provide a Payment Bond equal to 100 percent of the value of the Agreement. Contractor shall complete the State of California, Standard Form 807, in duplicate and submit prior to the execution of contract. Anticipated Term Date: Forty-Five (45) Days Contractor State License Board Requirement: C-15 Flooring and Floor Covering License. This acquisition shall be awarded pursuant to Government Code Section 14838.7 (Public Works) for contract awards of $5,000.01 to $333,000.00 to State of California certified Small Business / Disabled Veteran Business Enterprises. Bidders who fail to submit all of the requested documents, will be deemed non-responsive and their quote will be rejected.
