Furnishing all tools, materials and labor and performing the work necessary to reconstruct 3,100 linear feet of two lane collector asphalt pavement street. Project includes 6,420 square yards of cold milling asphalt pavement, 3,160 square yards of concrete base removal, 3,160 square yards of subgrade method B, 3,160 square yards of 8 inch Type A Aggregate base, 1,440 tons of asphalt pavement and 3,160 square yards of 7 inch dowel jointed HES PC Concrete pavement. Project also includes removal and replacement of approximately 400 square yards of 4 inch concrete sidewalk and removal and replacement of 575 linear feet of 30 inch combined curb and gutter. Multipolymer traffic striping and construction signing and traffic control are included in the project as well. The information for Bidders, Forms of Bids, Form of Contract, Plans, Specifications and Form of Bid Bond, Performance and Statutory Bond, and other contract documents may be examined at the Office of the City Clerk of Ponca City, Oklahoma. The successful bidder shall submit Certification of Insurance for Public Liability and Workers Compensation in the amounts prescribed by Statute with the City of Ponca City named as additional insured. Bids filed as provided herein shall be publicly opened on the above-mentioned time, date and place. All bids shall remain on file at least forty-eight (48) hours thereafter before a contract shall be made and entered into thereon. Bids received more than ninety-six (96) hours, excluding Saturdays, Sundays and holidays, before the time set for the opening of bids, as well as bids received after the time set for opening of bids, shall not be considered and will be returned unopened. All bids submitted must include the word BID, the name of the bid item or project, and the time and date of the bid opening on the face of the sealed envelope. A cashiers check, a certified check or a security bond in the amount of five percent (5%) of the bid shall accompany the sealed proposal of each bidder. Deposits will be returned to unsuccessful bidders. Where Vendor and Subcontractor Identification Questionnaires are included in the bid documents, each bidder shall submit the Questionnaire directly to the Engineer no later than 5:00 p.m. on the first working day following the bid opening.
