Price quotes for providing a Water Management Program for two Outdoor Aquatic Facilities IMPORTANT: Notes, Terms, and Conditions 1. All quotes must be submitted using the attached form, along with any necessary additional attachments. 2. Quotes need to be complete, address all services requested, and must be signed by vendor. 3. All quotes received after the specified date and time will be rejected. 4. Taxes must be excluded from quote. All quotes must be submitted without federal excise and Wisconsin sales taxes as the City of Waukesha is exempt from them. 5. Even if a quote is accepted, a purchase order from WPRF will still be required before commencing any services or delivering any goods. 6. WPRF may reject any and all quotes. 7. quotes should be submitted for each individual aquatic facility (Horeb Springs and Buchner); however, acceptance will be based on the total lump sum quote for both aquatic facilities. 8. Quotes must include unit prices for Pulsar Calcium or equivalent or equivalent (if equivalent is specified a free rental feeder must be provided), Hypochlorite, Acid Magic or equivalent, Sodium Bicarbonate, and Calcium Chloride. Water Management Program is for the two following City of Waukesha outdoor aquatic facilities: Horeb Springs Aquatic Center Buchner Park Community Pool 330 Spring Street, Waukesha WI 53188 223 Oakland Avenue, Waukesha WI 53186 Square Footage: 8777 sq. ft. Wading, Pool Square Footage: 1,760 sq. ft. Main Pool Square Footage: 9,854 sq. ft. Plunge Pool Square Footage: 2,446 sq. ft. The contract will be for an initial term of a two-year period (calendar years 2024 and 2025) and may be extended annually for additional one-year extension terms by the mutual agreement of the parties, upon the same terms and conditions except that the contract price may be adjusted. Notification of a partys desire to extend, including any price change request, shall be given to the other party at least 90 days before the expiration of the then-current term. The Vendor shall maintain a policy of public-liability insurance and a policy of professional errors and omissions insurance, each with limits of not less than $1,000,000 per occurrence and $3,000,000 aggregate, issued by an insurer licensed to do business in Wisconsin.
