Each proposal shall contain the full name and address of every person, firm or corporate interest in the same, and if a corporation, the name and address of the President and Secretary. Each proposal shall be accompanied by a certified check on a solvent bank or Bid Bond for the amount of $500.00 made payable to the City of Ironton, Ohio. The successful bidder shall forfeit his certified check or bid bond if said bidder fails to enter into an agreement with the City of Ironton based upon his proposal or fails to supply chemicals at any time.
