A notification of intent to respond to this RFP is located immediately below. This page must be submitted to notify Truman of your interest in this project and your plan to submit a proposal. This form is also required if you wish to receive (1) answers to questions regarding the RFP and (2) any RFP addenda. RFP addenda will be issued if there is a change to the specifications or closing date/time of this RFP. Truman seeks a single vendor to provide a new pre-engineered replacement pedestrian bridge located on Truman's campus in Kirksville, Missouri. This RFP contains specific information that must be addressed in the vendor's proposal response. These specifications will ensure the vendor furnishes the highest quality products and service. This RFP, and any subsequent addenda, constitute the specifications and requirements for this project. Any and all communications regarding specifications and requirements should be directed to Truman's buyer referenced in this RFP, and it is the vendor's responsibility to ensure any request for information is received by Truman's buyer. The vendor may suggest additions or enhancements to the following requirements. Additions or enhancements, along with their associated costs, should be clearly delineated as optional in the vendor's proposal. A. Required Specifications Unless specifically stated in the RFP, the following specifications constitute the minimum requirements for this project. 1. Bridge design will be in accordance with the American Association of State Highway and Transportation Officials (AASHTO) Guide Specification for the Design of Pedestrian Bridges, latest edition. 2. The bridge will be a truss style design and have the following approximate dimensions: a. Length (Span): forty-five feet (45') b. Interior Width (Deck): eight feet (8') c. Horizontal safety rails will be placed on the structure up to a minimum height of forty-two inches (42") above the deck surface. A four inch (4") sphere must not pass through the lower thirty-four inches (34") inches of the rail system, and an eight inch (8") sphere must not pass through the upper area of the rail system between thirtyfour (34') and forty-two (42") inches. 3. The bridge will be constructed of the following materials: a. The bridge's truss framework will be painted square and/or rectangular structural steel tubing. Other structural members and bracing will be fabricated from structural steel shapes, square and rectangular structural steel tubing or other high tensile strength materials. All components will be corrosion resistant. b. Decking will be fiber reinforced polymer (or similar composite), or steel floor grating that meets The American with Disabilities Act (ADA) requirements with a maximum opening of one-half inch ( 1/2 "). c. Associated hardware including, but not limited to, bolts, fasteners, splices, etc. will be high strength in accordance with the AASHTO specifications for structural joints. d. Where material options exist, Truman reserves the right to make final materials selections. 4. The bridge will support pedestrians and/or light vehicle traffic having a designed load rating of not less than sixty (60) pounds per square foot for a uniform live load and/or a light utility vehicle load of three thousand (3,000) pounds. 5. The vendor will provide two complete sets of measured bridge drawings (including any applicable proof of engineering certifications) and two complete sets of detailed assembly instructions. 6. If required, the vendor will provide either remote or in-person technical assistance to Truman throughout the assembly process. 7. The bridge will have an unconditional lifetime warranty against manufacturing defects and a not less than a five year warranty on all parts and components. 8. The vendor will provide written inspection and maintenance procedures to be followed by Truman. B. Required Shipping and Delivery 1. The vendor will make provision to deliver the bridge and all associated hardware to Truman's campus. 2. Bridge components will be delivered by truck to a location nearest to the site accessible by roads. Hauling permits and freight charges are the responsibility of the vendor. 3. The vendor will notify Truman in advance of the bridge's expected arrival. 4. The vendor will advise Truman of the actual lifting weights, attachment points and all necessary information to install the bridge. 5. The vendor, and/or any shipping subcontractors, will purchase and maintain the following insurance policies before entering Truman's campus: a. Comprehensive General Liability: Coverage (including products/completed operations liability insurance and broad form comprehensive general liability endorsement or its equivalent) will have minimum limits of $1,000,000 per occurrence and $3,000,000 in aggregate. b. Worker's compensation insurance as prescribed by the laws of the State of Missouri. c. The vendor, and any subcontractors, will provide Truman with certificates of insurance for all required policies within ten (10) days of award notification naming Truman State University as an additional insured party. Such certificates will provide that Truman be given at least 30 days prior written notice of any cancellation, intention to not renew, or material changes in these policies. Failure to provide, and continue in force, the insurance coverages required above will be deemed a material breach of the contractual agreement resulting in immediate termination of the contractual agreement. Presence of insurance coverage does not relieve the vendor of responsibilities or obligations assumed by the contractual agreement for which the vendor may be liable. 6. Truman will not be responsible for lost or damaged materials or components during transportation. C. Optional Assembly and Installation Truman, at its sole discretion, may consider on-site assembly and installation services associated with a pre-engineered bridge purchase. Vendors are not required to propose on-site assembly and installation services. However, if a vendor elects to propose such services, then the scope of services provided will be fully described in the vendor's proposal. The cost for such services will also be delineated separately in the vendor's pricing proposal. Assembly and installation services will be subject to the following provisions: 1. All labor, tools, and materials necessary for the completion of all work included in assembling and installing the pre-engineered pedestrian bridge will be furnished by the vendor at its own expense. All labor shall be performed in a first-class, substantial, neat and workmanlike manner. All damage to Truman's property will be repaired, and Truman's facilities will be left complete in every respect. 2. The vendor must locate and mark all utilities within proximity of the project and avoid damage to facilities 3. The vendor will install the new pre-engineered pedestrian bridge in accordance with the manufacturer's instructions. 4. The vendor will assume responsibility for any damage to the pre-engineered pedestrian bridge during assembly and installation. The vendor will correct any deficiencies resulting from assembly and installation. The vendor will keep the premises clean and orderly at all times. The installation site will be left clear of clutter and trash at the end of each work day. Any trash should be disposed of in Truman's receptacles. 6. The vendor, and/or any subcontractors, will purchase and maintain the following insurance policies before entering Truman's campus: a. Comprehensive General Liability: Coverage (including products/completed operations liability insurance and broad form comprehensive general liability endorsement or its equivalent) will have minimum limits of $1,000,000 per occurrence and $3,000,000 in aggregate. b.
