Lyons Township High School 2024 Asbestos Abatement Work will start June 3 and be phased through June and July. The work includes the following: Base Bid - South Campus - o 1st phase will be ACM pipe insulation removal in the E wing. Scope includes gross removal of all ACM pipe insulation in the tunnels as shown on the drawings. Alternate Bid 1 applies to this phase. AAC start on containments for next phase during clean period. o 2nd phase will be ACM pipe insulation removal in the G wing. Scope includes gross removal of all ACM pipe insulation in the tunnels as shown on the drawings. Scope includes glovebag removal on the 1st floor and above the first floor ceiling in mini-containments. GC will open chase walls and ceiling, AAC will abate. Scope includes intact removal of floor tile at B wing-G Wing connector and Storage Room S.B.1. AAC start on containments for next phase during clean period. o 3rd phase will be ACM pipe insulation removal in the D wing. Scope includes gross removal of all ACM pipe insulation in the basement storage area and subsequent glovebag removal of pipe insulation in mini-containment in tunnel south of gross removal area. Scope includes glovebag removal on the 1st floor and above the first floor ceiling in mini-containments. GC will open chases and walls (assumed to run piping up to fixtures and into ceiling) and ceiling, AAC will abate. Scope includes removal of interior door sealant in a mini containment. o Exterior Doors (2) as indicated at the D wing abatement area. May be done from the exterior as EPA work. Coordinate with GC. o Exterior and interior door sealant as indicated at the north end of the B wing. Requires containment for interior doors. Coordinate with GC. Alternate Bid 1 - Phase 1, South Campus - o Glovebag work at four bathrooms as identified on drawings. GC will open chase walls, Abatement Contractor (AAC) will set up mini-containments, abate, area will be cleared and AAC will take down containments. Work includes a 2-story stack between 2 restrooms and glovebag work in the tunnels below at connections and 2' either side on mains. Alternate Bid 2 - North Campus - o Removal of exterior door sealant at multiple locations as identified on the drawings. All Bidders are requested to attend this meeting and to participate in a walk-through of the facility. Bids shall include: 1. Properly completed Proposal Form in duplicate. 2. Not less than the prevailing hourly wage rate as determined by the Illinois Department of Labor pursuant to Illinois Revised Statues, Ch. 489, Sec. 39s-1 et.seq. 3. Bid Security in the amount of 5% of the bid. All Bidders are advised that the law concerning prevailing wage rates must be complied with. Prevailing wage rates have been determined and are on file at the office of the State of Illinois, Department of Labor or on their website. Bidders must be prequalified with the State of Illinois, Department of Public Health and meet the requirements of the latest edition of the Illinois Asbestos Abatement Act and Rules and Regulations. Bidders must be prepared to submit evidence of prequalification with the Department of Public Health and a current audited financial statement to the Office of the Superintendent upon request of the Owner or Specialist. All Bidders shall submit and enclose with their bid a certification that the Contractor is not barred from bidding on the Contract as a result of a violation of either Section 33 E-3 or 33 E-4 of Article 33E, Public Act 85-1295 (Senate Bill 2002) effective January 1, 1989. Certification is located in the Proposal Form, Section 00300, of the Specifications. Bids shall be accompanied by a bid bond, certified check or cashiers check in the amount of 5% of the bid made payable to Lyons Township High School. Failure of bidder to submit bid bond, certified check or cashier's check in amount to cover proposals bid upon is cause for rejection of bid. Successful Bidder will be required to provide Surety Performance Bond and Labor and Material Payment Bond, each equal to 100 percent of the contract price on prescribed forms within 5 days after notice of acceptance is issued by Owner. Surety on bonds shall be by duly authorized company acceptable to the Owner. The Owner reserves the right to reject any or all bids and reserves the right to waive any irregularity in bidding, which in their considered opinion is warranted.
