This form shall be submitted regardless of the bid total. Bidders who fail to submit the required disclosure form will be considered non-responsive, and their bid will not be considered for award. The disclosure form shall either be submitted in the same envelope as the bid, or shall be submitted in a separate sealed envelope clearly marked "Disclosure Form for the Water Storage Tank Recoating Project," showing the date and time of the disclosure submittal deadline, as well as the Bidders name. The work includes the full removal and replacement the interior coating system in an existing 2 million gallon steel potable water storage tank as well as the installation of cathodic protection system. Any prospective bidder not attending the pre-bid open house will be fully responsible for making separate arrangements with the City of Waldport for access to examine the project site. In the event that the prospective bidder fails to coordinate such separate access with the City of Waldport, the bidder will be fully responsible for any and all information affecting the cost, progress or performance of the work that could have been obtained by such a pre-bid inspection visit. This contract is for a public works project subject to ORS 279C.800 to 279C.870 (state prevailing wages).
