Site work and paving for a transportation facility in Spanish Fork, Utah. Completed plans call for site work for a transportation facility; and for paving for a transportation facility.

The project consists of but is not limited to pavement marking removal, crack sealing, seal coat application and re-marking airfield pavements to include: Runway 12/30, Taxiway A from A4 to A6, and connecting Taxiways A1 through A6 (approx. 90,000 SY). Each bid must be accompanied by a certified check, cash, cashier's check, or bid bond in an amount not less than 5% of the total bid. Spanish Fork City Reserves the Right to Reject Any and All Proposals, Waive Any Informalities, or Irregularities, Postpone the Award of the Contract for a Period Not to Exceed One Hundred Twenty (120) Days, and Accept the Proposal That is in the Best Interest of Spanish Fork City. The Award of the Bid is Contingent Upon the Receipt of Federal Funding. Spanish Fork Municipal Airport is a general aviation airport in north central Utah. It is located at 4008'42.13"N latitude and 11140'03.73"W longitude. Spanish Fork Municipal Airport is owned and maintained by the Spanish Fork City (Sponsor). The airport is the home base for 237 aircraft, a fixed base operator (FBO), and multiple flight schools. The airport is currently an Airport Reference Code (ARC) B-II airport serving a mix of single engine piston, multi-engine piston, jets, and helicopters. Spanish Fork Municipal Airport consists of a paved Runway 12/30, a full-length parallel Taxiway A with 6 taxiway connectors, Taxiway B1 and a general apron. Runway 12/30 is equipped with runway threshold and medium intensity edge lights. Additionally, both ends of Runway 12/30 are equipped with 4-box PAPI units. Ancillary equipment on the airport includes a lighted wind cone, an AWOS, and a rotating airport beacon. Project Description The Airfield pavements are starting to exhibit signs of oxidation and cracking. Pavement preservation will consist of crack sealing, pavement marking removal, and application of a seal coat, along with remarking of existing pavement markings. This rehabilitation will reduce the chances of future pavement deterioration and help prolong the life of the pavements. Construction for this project is scheduled to occur in FY 2024. CSPP: Purpose and Process This Construction Safety and Phasing Plan (CSPP) has been developed in accordance with FAA Advisory Circular AC 150/5370-2, current edition, as part of a process to assess and minimize safety hazards at Spanish Fork Municipal Airport during this project. The CSPP will serve as the Sponsor's documentation of the foreseeable hazards, assessment of risks involved, and controls used to mitigate that risk. This document will ultimately be accompanied by one companion document, drafted by the Contractor, known as the Safety Plan Compliance Document (SPCD). The SPCD details how the Contractor will comply with the CSPP and will include Contractorspecific information that will only be available from the successful Contractor. Recommendations made in the CSPP will be incorporated in the SPCD and will be enforced during construction. Additionally, the SPCD will include contractor specific information needed for providing notice of construction activities to the FAA and the Sponsor.
