Certified, Treasurer's or Cashier's Check or bid bond in the sum of 10 percent of the bid, but in no case in excess of $20,000, Agreement of Surety, Surety Disclosure Statement and Certification (if bid amount is greater than or equal to $800,000), List of Subcontractors, Ownership Disclosure, Bidder's Affidavit, Non-Collusion Affidavit, References, Acknowledgement of Receipt of Changes, Bidders Check List, and Questionnaire are required with each Bid. Successful bidder must furnish 100 percent Performance Bond and 100 percent Labor and Materials Bond. The work includes the removal and proper disposal of the grit in the Sludge Storage Tank and all appurtenant work required as shown and specified in the bid documents.
