Combination cementitious and epoxy lining of approximately 431 vertical feet of sanitary sewer manholes, chemical grouting of joints and cracks as required for manhole lining, installation of spray on urethane internal chimney seal material for 33 manholes, removal and replacement of one (1) manhole casting, traffic control and other miscellaneous associated work. All bids shall be made on the form furnished by the City through wdm.ionwave.net and shall be filed on or before the time specified above. Each proposal shall be electronically accompanied by a scanned copy of a bid bond, or a scanned copy of a cashier's or certified check drawn on an Iowa bank or a bank chartered under the laws of the United States in an amount equal to ten percent (10%) of the total amount of the bid. If a bid bond is submitted, it must be on the form provided with the contract documents. A physical copy of the bid security will be required to be submitted to the City Clerk's Office by the apparent low bidder within 48 hours of the bid deadline The successful bidder will be required to furnish a performance and payment bond in the amount of one hundred percent (100%) of the contract price guaranteeing faithful performance of the contract and guaranteeing payment to all persons supplying labor and/or materials in the execution of the work provided for in the contract. Additionally, the contractor must provide the City with a guarantee of maintenance of said improvement for a period of four (4) years from the time of acceptance by the City. Work on said project shall commence within ten (10) days of a written notice to proceed and shall be completed by September 27, 2024. Liquidated damages in the amount of five hundred dollars ($500.00) per day will be assessed for each day that the work remains uncompleted after each division period and the final contract period.
