Diversey Northlake, Il 60164 The envelope shall be addressed to Mannheim School District 83 and shall carry the project name, the bidder's name and address and the bid package designation for which the bid is being submitted. A complete bid will include the bid form, bid bond, and signed non-collusion statement. All interested bidders must demonstrate previous experience with repair & site work in State of Illinois schools in a confidential AIA 305 Statement of Qualifications.
