The Tarrant County Hospital District d/b/a JPS Health Network (the "District") is seeking proposals for the provision of Operating Room Floor Renovations. The District is requesting proposals from qualified vendors to provide labor, material, equipment, and supervision required to install 3/8" epoxy terrazzo flooring with a one-color pattern as well as a one (1) year warranty on the flooring materials. Furnish all necessary labor, material, equipment, and supervision required to install 3/8" epoxy terrazzo flooring with a one-color pattern. Full coverage moisture mitigation system. Crack isolation membrane. 1/8" zinc divider strips. 6" poured-in-place coved base. 13 OR rooms - 3 rooms per mobilization. Demo of existing Stonhard product Floor prep and leveling Construction containment Work to include nights and weekends. The District maintains a policy of encouraging and engaging in business transactions with vendors who qualify and are certified under applicable law as Minority, Woman, and Veteran Owned Business Enterprises ("MWVBEs"). The District establishes a 25% good faith target goal. The proposed term of the contract is three (3) years with two (2) additional one-year renewal options.
