The work under this Work Document-Driven Contract consists of removal, replacement and/or repair of ditch pavement, driveways, sidewalks, detectable warning surfaces (DWS), curbs, gutters, drainage pipe, mitered end sections, guardrail asphalt pads, in Leon and Gadsden Counties, as outlined in the Appendices. Other services required to achieve this work include Maintenance of Traffic (MOT), adjusting utility features (e.g. manhole rings, water meter boxes, valve risers, etc.), providing erosion control features, borrow material and hot-mix asphalt, maintaining pedestrian and driveway access, sodding and watering of disturbed areas. Coordinate work with owners of nearby affected utilities. Contract services will be requested on a call out basis. This Contract has a budget estimate of $600,000.00. Mobilization is incidental to other pay items listed. For bids over $150,000.00, the standard proposal guaranty of 5% of the bid will be required, unless otherwise stipulated in the proposal advertisement. A Proposal Guaranty of not less than five percent (5%) of the total actual bid in the form of either a certified check, cashier's check, trust company treasurer's check, bank draft of any national or state bank, or a Surety Proposal Guaranty made payable to the Florida Department of Transportation must be received for each bid in excess of $150,000.00. A 100% Payment and Performance Bond will be required for all projects. All work is to be done in accordance with the Plans, special Provisions of the State of Florida Department of Transportation.
