This work is estimated by the Department of Administration, Public Works Division to be less than $150,000.00. Bid must comply with the below listed requirements and utilize DAPW Form 35. Prequalification is not required. The Contractor shall provide, on a quarterly basis, all labor, material and supervision for a preventive maintenance service as specified herein for the Building Automation System. Work shall be bid as a lump sum amount for 2 years. Emergency Services is included in the contract. All scheduled on site preventive maintenance work shall be performed Monday thru Friday, between the hours of 8:00 AM and 3:30 PM The work to be performed under this agreement shall be for a 2-year period for the dates as stated in the fully executed contract. The length of the agreement is seven hundred and thirty (730) days and is renewable upon concurrence of all parties for an additional two (2) contract years. All invoices shall have the Purchase Order number clearly stated on them. Failure to do so may result in returning of invoices for resubmission and or lack of payment. Payments will be made quarterly in arrears. The quotation form, and any other forms which may be required by the specifications, must be submitted in a single sealed envelope. The envelope must be plainly marked with the name of bidder, project description, requisition number, bid time, and bid date. Quotation shall be rejected if all required documents are not in the single sealed envelope, or if the envelope does not identify the material as a quotation. If he finds it in accordance with the bid documents, it is to be returned to Public Works Division by certified mail or in person within ten (10) calendar days after receipt for further execution and with the caution that a contract will not exist until it is signed by all signatories required. No right conferred on either party under this Contract shall be deemed waived, and no breach of this Contract excused, unless such waiver is in writing and signed by the party claimed to have waived such right. Neither the States review, approval or acceptance of, nor payment for, the services required under this Contract shall be construed to operate as a waiver of any rights under this Contract or of any cause of action arising out of the performance of this Contract, and the Contractor shall be and remain liable to the State in accordance with applicable law for all damages to the State caused by the Contractors negligent performance of any of the services furnished under this Contract
