Consent of Surety in total amount bid, must also be submitted with the Bid Proposal. A Bidder on a Public Works Project for a Board of Education where the cost of the Work exceeds $20,000.00 must first have been qualified by the Department of the Treasury, Division of Building Construction, pursuant to NJSA 18A:18A-27 through 33, and shall submit with his Bid Prequalification Affidavit, a copy of a valid and active Notice of Classification, a certified copy of the Total Amount of Uncompleted Contracts form, and Affidavit of No Material Adverse Change. All Contractors named in this proposal shall possess a valid Public Works Contractor's Registration Certificate pursuant to NJSA 34:11-56.48 et. seq., at the time the proposal is received. No Bid Proposal may be withdrawn for a period of sixty (60) days after the date of opening. Bidders are required by law to submit a list of names and addresses of all interest holders owning 10% or more of the business entity's interest. Contractors bidding on this Project shall comply with the requirements of the Prevailing Wage Rate Determination pursuant to N.J.S.A. 34:11-56.27 et seq. Inspect and Replace Any Corroded Decking on a Unit Cost Basis With Approval of Engineer. 3. Install R-30 Insulation and 2 Ply Modified Bitumen Roof System Per Roof System Component Chart on S-000. 4. Install All New Scuppers, Flashing and Accessories to Provide 20 Labor on Materials Substrate. 5. Install Interior Drains at Identified Locations (Alternate Bid). 6. Reconstruct Masonry Parapets as Identified. 7. Perform All Masonry Repairs Per the Drawings. 8. Application of a Water Repellant on All Masonry Surfaces. 9. Replacement of Canopy Roof Assemblies.
