PURPOSE OF REQUEST FOR STATEMENT OF QUALIFICATIONS (RFSQ) San Juan Health Service District (District) is requesting qualifications from Architectural firms to provide the design and related architectural services for a replacement critical access hospital building in Monticello, Utah. SCOPE OF WORK Design of a new Critical Access Hospital located in Monticello, Utah (Approximately 42,000 sq. ft.). Architectural Services shall include, but may not be limited to those stated in the standard form of agreement between Owner and Architect, AIA Document B101. Additional services may be requested or required by San Juan Health Service District for this particular project. Examples of additional requirements may include: submitting documentation and acquiring approvals from local, state and federal agencies necessary for hospital licensure.
