The Burrillville Housing Authority (BHA) is inviting all interested parties to submit a proposal for a 3 year contract with 2 year extension option for painting interior apartments at Ashton Court. Use the form provided. Use of any other form will be reason for disqualification of the bid. SCOPE OF SERVICES: The selected contractor will be responsible to provide all material, equipment and labor to complete the work specified below. Properly prepare all surfaces prior to applying paint. Preparation is to include minor patching which is defined as, but not limited to, holes from nails and screws or indentation from door knobs, etc. Primer is to be applied over the areas as required. Apply one coat (or two if needed) of paint on the walls. Primer does not count towards the one coat requirement. Paint will be Dover White, Eggshell Sherwin Williams Promar 200 or equivalent. Apply one coat (or two if needed) of paint to the textured ceilings. Primer towards the one coat requirement. Paint will be flat white Sherwin Williams Promar 200 or equivalent. In occupied units, all walls, ceilings will be painted. This does not include the closets. If previously painted, the trim and doors will be included. All natural or clear finished surfaces will not be included. The contractor is responsible for removing and replacing all decorations, move all furniture as necessary to complete the work. Furniture will be moved back to its original location when work is completed. All items will be covered with drop clothes. The contractor is responsible to repair or replace all damage caused by the contractor. All units and surrounding areas are to be left neat and clean. GENERAL INFORMATION: The Housing Authority will do a minimum of twelve (12) units during the contract year. At the rate of a minimum one (1) unit a month. The Housing Authority will provide a minimum of a 48-hour notice for painting. This is a residential prevailing wage project. The wage rate in use on the date of the Contract signing will govern. All apartment painting is to be completed in a timely manner with no more than one working day in each apartment. Any contract resulting from this Proposal may be canceled with a one (1) week notice from the Housing Authority to the Contractor or with a one (1) week notice from the Contractor to the Housing Authority. INSTRUCTIONS TO CONTRACTORS: Proposals are to be submitted in an envelope clearly marked with the title of this Proposal noted in the lower left-hand corner of the envelope. The Contractor is to provide proof of in force Workman's Compensation and General Liability insurance for all employees, equipment and vehicles, regardless of whether the vehicle is company owned or private. Proof of insurance must be on file at the Authority's office prior to the start of any work. Bidder is to include with his bid three (3) references, including name and telephone number.
