Install 1,650 LF of 8" DIP and all appurtenances. Renew 101 inlet services. 1. Please note that documentation of pressure and leakage testing in accordance with GW Specification Section 4-15 will be required.
2. Project to be under construction within 2 weeks of issuance of contract unless otherwise communicated with GW and Engineering Department.
3. Seven (7) compaction tests required at locations to be specified by GW Inspector. 98% compaction required under paved surface and 95% compactionrequired along ditch sections per Permit Specs. Percent compaction, percent moisture, wet density, and dry density to be provided with compaction report.
4. Contractor required (per Encroachment Permit Specs) use screenings on roadway if placing excavated spoils on road surface.
5. Performance Bonds and Payment Bonds on GW standard forms required with GW Contract.
6. All items related to Milling and Full Width Resurfacing, as outlined in the
encroachment permits, is part of this contract.
7. Unless noted, all work shown on drawings is require whether or not a specific
bid item is listed. Payment for work for which a bid item is not listed shall be
included as part of other bid items.
8. Retainage of 10 percent will be deducted from each pay request until project
completion and final acceptance of all work performed.
9. Within 10 days of written notice of acceptance of this bid, Contractor will
execute the contract and deliver the bonds and insurance certificates to GW.
10. Contractor will be required to flush the mains p e r t he c on stru c tio n no t es
as required by GW. Contractor will also be required to furnish and install flow
meter prior to flushing.
11. If awarded large taps and tap-overs the Contractor will be required to attend GW Tap Training prior to performing any taps. Contractor will also be required to submit pictures of all large taps; all service line taps and all tap-overs prior to meter box installation. If large taps and tap overs are not performed correctly and as instructed during tap training the Contractor will not be allowed to perform large taps and tap-overs on this project or future projects. GW will decide which alternate bid items will be installed by Contractor or will be installed by GW for all alternate bid items.
12. Contractorshall submit the two attached affidavits with his bid: Ethics in Public Contracting Af idavit Non-collusion Af idavit of Prime Bidder
13. Contractor will be required to sign a GW release and waiver of lien before final payment is made.
14. Failure to complete all work by August 1st 2024, date will result in liquidated damages in the amount of $500 per calendar day. Liquidated damages will be deducted from pay requests.
15. It will be the responsibility of the contractor to ensure that the required notice permit provided is on the job site at all times during work.
16. Any US Pipe from Lynchburg, VA used on job site will only be accepted if cut sample meets minimum AWWA concrete liner thickness requirements. One piece per batch to be cut for field inspector to verify if
requirements are met.
17. Contractor to begin construction on Druid St. (GV), followed by Windsor Dr (GV), and Perry Rd. (S-23-188). Construction not to impede Greenville City or SCDOT Repaving. Either Reid St. (GV) and Tabor St. (GV) can be replaced after Perry Rd.
18. Perry Rd. (S-23-188) must be completed by August 1 st 2024 per DOT Permit. Work not completed will be removed from the project and omitted from the final invoice.
