The Department of Veterans Affairs, Veterans Health Administration (VHA), Network Contracting Office 20 (NCO 20) is conducting a market survey and is seeking potential sources for Switch Repair This Sources Sought Notice is issued for information and planning purposes only. This is not a solicitation or a request for proposal and shall not be construed as an obligation or commitment by the Government. An award will not be made on any offers submitted in response to this notice, and it shall not be implied the Government is committed to providing any solicitation or award following this notice. The Government will not pay for any information received in response to this request, nor will the Government compensate a respondent for any costs incurred in developing the information provided.   Responses shall be submitted to Nick Price at Nicholas.Price@va.gov by COB 04/08/2024 at 17:30 PST SUBJECT: RFI 36C26024Q0456 switch repair  This notice is intended strictly for market research. The purpose of this Sources Sought Notice is to determine interest and capability of potential qualified sources of supply and determine the socioeconomic size classification of the supplier and manufacturer of the end item.  Interested companies shall provide, at a minimum, the following information with their response;   Company Name and Address: Point of Contact (POC) Name: Email Address: Phone Number: DUNS Number/UNIQUE SAM ID: Brochures of their offered equipment.  The anticipated North American Industry Classification System (NAICS) 561621. Mark if your firm is eligible for participation in one of the following small business programs. The Government will in no way be bound to this information if any solicitation is issued. STATEMENT OF WORK (SOW) VA SORCC Statement of Work Repair and test ATS B211 Rm. 178 A. GENERAL A.1 GOAL: Contractor shall provide White City, Oregon, VA Southern Oregon Rehabilitation Center and Clinics (VA SORCC) Labor and parts to repair and test the Automatic Transfer Switch in building 211LS These subject items need to be tested, (exercised) in accordance with NFPA 110. A.2 LOCATION OF SERVICES: VA SORCC 8495 Crater Lake Hwy White City, OR 97503 A.3 SCOPE Items to be Repaired and site location: Zenith Automatic Transfer Switch in B211 Rm. 178 B CONTRACTOR B.1 CONTRACTOR DELIVERABLES Contractor shall provide a list of all repairs done and a functional test report. B.2 CONTRATOR SORCC SITE CHECK IN: The Contractor employees shall check-in at SORCC Building 229 (FMS) and check out when complete. If after hours, contact VAPD in building 202. Contractor shall wear a VA identification badge at all times while on station. Badge will be obtained in building 229 when signing in and shall be returned upon completion of the services when signing out. B.3 CONTRACTOR PERSONNEL The contract manager shall be responsible for management and coordination of the contract and shall be the point of contact with all government representatives. The contractor shall notify the contracting officer in writing prior to contract start date the names of the Contract Manager and an alternate contract manager. The Contract Manager shall be available for the Government to reach. During normal business hours, the contract manager shall respond within one business day from the time of notification. During non-business hours, the Contract Manager shall respond within one business day. C SPECIAL CONTRACT REQUIREMENTS C.1 QUALITY CONTROL The contractor is responsible for developing and maintaining a quality program to ensure that the services are performed in accordance with commonly accepted commercial practices. The contractor is responsible for developing and implementing procedures to identify, prevent, and ensure non-recurrence of defective services. At a minimum, the contractor is responsible for developing quality control procedures addressing each specific bid item of the contract to include a service delivery summary. The contractor is responsible for submitting the quality control program to the Contracting Officer with their proposal. C.2 QUALITY ASSURANCE 1. Contractor will be here on the appointed times and days, unless necessary to come more often as requested by the VA SORCC. 2.The completed services shall be performed in an efficient and reliable manner. The VA SORCC has the right to reject work if the work is not performed in an efficient and reliable manner with the timelines and frequencies outlined above. C.3 RECOGNIZED HOLIDAYS The contractor is not responsible to provide services on the following days but must provide frequent enough service prior to and after these holidays: New Years Day, Martin Luther Kings Day, President s Day, Memorial Day, Fourth of July, Labor Day, Columbus Day, Veteran s Day, Thanksgiving Day, and Christmas. C.4 PERSONNEL POLICY The Contractor shall be responsible for protecting the personnel furnishing services under this contract. To carry out this responsibility, the contractor shall provide the following for their personnel: Worker s compensation, Professional Liability Insurance, Income tax withholding, and Social Security payments. The parties agree that such personnel shall not be considered VA employees for any purpose and shall be considered employees of the Contractor. C.5 LAWS & ORDINANCES The Contractor shall comply with all applicable laws, ordinances and regulations (Federal, state, city or otherwise) covering his operations under the contract. C.6 INVOICES All payments shall be made in arrears upon receipt of a proper invoice.
