Summit County is soliciting Statements of Qualification and Experience ("Proposal") from qualified firms ("Respondent" or "Respondents") with significant experience in providing professional land surveying services for the County in connection to the County's Recorder/Surveyor fulfilling requirements as set by the State of Utah. No guarantee of actual service requirement is implied or expressed by this RFP or the resulting agreements. Service requirements will be determined by actual need. The right is reserved by the County to reject any or all proposals, to waive any informality or technicality or to accept proposals deemed in the best interest of the County. All proposals that meet, exceed, or are comparable to minimum specifications will be accepted. The County is requesting proposals from skilled and qualified firms and individuals to providing professional land surveying services for the County in connection to the County's Recorder/ Surveyor fulfilling requirements as set by the State of Utah. The County shall be the sole judge as to which proposal constitutes the "lowest and/or most responsible proposal". An award will be made after the appropriate approvals are received. Interviews may be conducted to assist with the final selection. The candidates will be evaluated on their experience and abilities in all areas related to the professional land surveying and how these impact the needs of the County. In your proposal, indicate with particularity your unique abilities to serve the needs of the County. In addition, please specify all surveying-related services and programs provided by your organization and the cost and availability of those services. The County anticipates that the selected Respondent will be retained on an annual service agreement, renewable by the County on an annual basis for up to five (5) additional years. As work is anticipated on a "as needed, when needed" basis, work on a "fee for services" basis. Respondents must respond with service fees as shown on the Fee Schedule Matrix included in the Submittal Requirement section of this RFP. In addition to the foregoing, Respondents must respond to the list of requests for information found in this RFP. Additional information and/or materials may be attached that provides meaningful information about a Respondent. Avoid glossy brochures and other stock material that is not relevant to our specific needs and requests. The selected Respondent shall provide professional land surveying services and acts as a general advisor with knowledge of the best industry practices and provide advice and information regarding industry issues as it pertains to the surveying requirements and goals of the County. The selected Respondent should be knowledgeable of all applicable federal, state and local statutes, regulations, and ordinances. Services provided by the selected Respondent will be developed on an individual task basis. The selected Respondent and the County Recorder-Surveyor, or their designee, will develop an estimated budget for specific tasks before assigning any work. The selected Respondent will be expected to be qualified in performing a broad range of surveying services including, but not limited to, the following: Under the direction of the County Recorder/Surveyor, coordinate the proper replacement or restoration of survey monuments as established by UCA 17-23-13, UCA 17-23-14, UCA 17-23-17 and UCA 17-23-17.5. Provide other miscellaneous office and/or field surveying services, as needed by the County or the County Recorder/Surveyor. In order to be considered for this bid Respondent must complete a fee schedule matrix and answer the requests for information as follows: FEE SCHEDULE MATRIX SERVICE PROPOSED FEE PER REQUEST Establish section corners or controls as it relates to the public land survey system $ Confirm monument coordinates through tie sheets $ Replacement and restoration of disturbed surveying monuments as requested by the County. $ Office Hourly Rate for other surveying services as requested by the County. $ Field Hourly Rate for other surveying services as requested by the County. $ REQUESTS FOR INFORMATION State the exact name of the business entity that will provide any of the services you are proposing to provide to the County. Give the business address of the company. Specifically state the services that you propose to provide for Summit County. Please identify services other than those requested/specified herein that you believe may be of value to Summit County. State the name(s) of the management team that will be used in providing these professional services to the County during the term of any service agreement. Give the business address of each person named. Specify the education, work experience, background and professional credentials of each of the individuals named in response to #3. Specify whether you are willing to agree that each of the individuals specified in response to question #3 will not be replaced as one of the County's service "team" without the prior, written consent of the County. Please state the reasons why your company is the best qualified to serve the interests of the County. Provide a sample Certificate of Insurance for your company specifying the types and amounts of insurance protecting your firm and the County.
