JOB DESCRIPTION/SCOPE OF WORK RIVER ROAD: Top coat approximately 4,300 feet long by 20 feet wide with state spec. 1/2 inch wear course pavement 1 1/2 inches thick compacted. There are 4,900 square feet of driveway aprons. Existing paved driveways transitions will need to be milled. Shoulders will need 1 foot to 18 inches wide gravel placed by machine method with 3/4 inch ledge pack. Dirt driveway transitions will need to be blended with 3/4 inch ledge pack. There is one bridge that will need to be cold planed, it is 33 feet long by 22 feet wide. CENTER HILL ROAD: Top coat approximately 2,457 feet long by 22 feet wide with state spec. 1/2 inch wear course pavement 1 1/2 inches thick compacted. There are 2,567 square feet of driveway aprons. Existing paved driveway transitions will need to be milled. Shoulders will need 1 foot to 18 inches wide gravel placed by machine method with 3/4 inch ledge pack. Dirt driveway transitions will need to be blended with 3/4 inch ledge pack. MEADOW LARK LANE: Top coat approximately 937 feet long by 20 feet wide with state spec. 1/2 wear course pavement 1 1/2 inches thick compacted. There is a 50 foot by 20 foot hammer head turn around at the end of the road. There are 325 square feet of driveway aprons. Existing paved driveway transitions will need to be milled. Shoulders will need 1 foot to 18 inches wide gravel placed by machine method with 3/4 inch ledge pack. Dirt driveway transitions will need to be blended with 3/4 inch ledge pack. Signage, traffic control, and sweeping will be the responsibility of the contractor awarded the bid. All contractors must be pre-qualified with NH DOT to bid on this project. ADDITIONAL REQUIREMENTS: The Contractor agrees that it will carry any and all insurance which will protect it, the Town of Epsom and their officials, agents, volunteers and employees from any and all claims and demands, costs, damages, loss of service or consortium, expr related to the Contract. The Contractor further agrees that the Town of Epsom and its officials, agents, volunteers and employees shall be named as an additional insured in any and all such liability insurance policies required by the Town of Epsom., compensation, and attorney's fees including but not limited to any and all claims for personal injury and/or death, workers compensation injuries, and property damages which may in any way, arise from or out of the operations of the Contractor whether such operations be performed by the Contractor itself, anyone directly or indirectly employed by it or any other persons or company retained in any way to carry on all or portion of the operations, activities or work required by or related to the contract.
