Site work and paving for a mixed-use development in Otisville, Michigan. Completed plans call for site work for a educational facility; for paving for a educational facility; playground / park / athletic field; and playground / park / athletic field.
The LakeVille Community School District's Board of Education is accepting Sealed Bids to provide reconstruction to the pole vault runway at LakeVille Memorial High School. Proposed work includes site work, concrete, field event construction, and track surface. Questions addressed ONLY to Foresite Design during Bidding Period. Failure to follow instruction may lead to rejection of Bid. All materials and supplies incorporated and used in construction of the Work and becoming a permanent part of this Project will not be exempt from State Sales Tax and/or Use Tax. All required Sales Tax/Use Tax shall be included in the Bid price. Bids shall be accompanied by a sworn and notarized statement disclosing any familial relationship that exists between the Owner and any employee of Bidder and any member of the Board of Education or the superintendent. Additionally, Bids shall be accompanied by a sworn and notarized statement for the Iran Economic Sanctions Act Affidavit of Compliance a sworn and notarized Criminal Background-Sex Offender Affidavit. Bids not accompanied with these sworn and notarized statements will not be accepted by the Board. The LakeVille Community School's Board of Education reserves the right to accept or reject any and all Bids, either in whole or in part, to waive any informalities or irregularities therein, or to award the contract to other than the contractor(s) submitting the best financial Bid (low bidder), in its sole and absolute discretion. Failure to follow instruction may lead to rejection of Bid. Requirements of items included under General Work to be completed by all contractors. 2. Provide engineering and layout as required to complete this work. 3. Coordinate work with other trades on site. 4. Strict enforcement of this contractor's requirement to provide timely clean-up, removal and disposal of all rubbish and debris generated by this trade during the work. Maintain a clean condition at all areas on site and free from dirt, mud, and gravel. 5. Provide all permits required as part of this work. 6. Provide and install complete soil erosion control measures, including permits, bonds and maintenance. Maintain system including system log and remove system at the completion of the project or until approved to remove by governing authorities. 7. Protect existing asphalt/concrete from damage by equipment. 8. Remove and legally dispose of all excess materials and debris generated by scope of work, including topsoil, unless noted to stockpile. 9. Provide necessary dewatering associated with this work division. 10. Provide temporary pavement measures for vehicle or walkway traffic. 11. Furnish and install temporary track protection over existing track and asphalt. Upon completion of work on interior of track, Contractor shall remove track protection and powerwash / clean debris on existing track. 12. Provide temporary removal of existing perimeter chainlink fence for site access. Upon completion of all work, re-install chainlink fence. . Provide all labor and equipment necessary for the complete site demolition as shown within the Construction Limits unless specifically noted to be completed by others. Work includes but not limited to: landscaping, concrete, asphalt, stone, track surfacing, topsoil and unsuitable soil. Prepare and proof roll sub-grades, including sub-grade for paving and site concrete contractors. Work includes necessary backfill from demolition operations. 14. Prepare and proof roll sub-grades, including sub-grade for paving and site concrete contractors. 15. Perform earthwork as necessary to establish grades, and site grading to within 0.1 feet of the sub-grades indicated. 16. Provide all earthwork as shown. Work includes but not limited to rough grading, compacting and re-compacting, subgrade fine grading and positive drainage swales to existing and proposed catch basins as shown on drawings. 17. Provide labor, materials and equipment necessary for a complete installation of concrete paving and new sand / aggregate stone base. 18. Protect finished asphalt from excessive damage while completing specified work, including limiting dirt and debris onto surface. 19. Furnish and install new vault boxes. 20. Furnish and install track surface and markings. 21. Furnish and install new irrigation to provide coverage of newly restore areas. 22. Provide new topsoil and lawn restoration on all disturbed lawn areas. Work inside track shall include topsoil and sod; work on exterior of track shall include topsoil and seed. 23. Provide electronic copies of submittals as noted in specifications. If submittals comply with specifications, "Material Compliance Certificate" shall be used. 24. Provide all required closeout documents upon completion of project.
