The qualifications statement shall be no more than four (4) typed, single sided 8 1/2 " x 11" sheets in length. Anything in excess of this page limitation will not be read or considered. Resumes of proposed key personnel must be attached to the qualifications statement. Each resume shall be a maximum of two (2) single-sided 8 1/2 " x 11" sheets in length and should highlight education, professional credentials, and work performance on projects similar to that described below. The resume of the proposed project manager, as well as the resume(s) of proposed key technical staff must be included. Also included shall be a listing of work areas which the Consultant would subcontract out to other firms, the names of each subconsultant, summarized on one (1) page and the resumes of the subconsultant's(s') project manager(s). One (1) original, unbound, and four (4) bound copies of the qualifications package must be in a sealed envelope, labeled with the project name, project number and the name and address of the Consultant. Introduction The intent of this Request for Professional Qualifications is to select the leading candidates to provide inspection and contract administration Consultant services for the duration of the project described below. Provide a minimum of a full-time Resident Inspector under the supervision of a Consultant Project Manager. The Project Manager shall be a Professional Engineer licensed in the State of New Jersey and shall make periodic site inspections as required. The Consultant firm's office support staff shall include qualified professional engineers, and technicians, as needed, for shop drawing reviews, responses to Contractor "requests for information", resolution of unforeseen circumstances and other project support functions. Consultant services shall also include scheduling and coordination for materials sampling and testing. These sampling and testing services shall be performed by a Certified Laboratory under a separate annual on-call testing contract awarded by Monmouth County. No testing or sampling fees should be included in this Consultant's scope of work; however, the Consultant should include cost of coordinating with and reviewing the results of the on-call testing Consultant. Please note that this Project's Design Consultant (HNTB Corp.) has been retained to provide design support services during the construction phase, such as "design intent" shop drawing reviews and RFI responses. All other shop drawing reviews and RFI responses will be performed by the Consultant providing the inspection and contract administration services Project Description The Consultant firms(s) shall provide the County of Monmouth with professional contract administration and inspection services necessary for the Bridge W-43 Fender Replacement on County Route 18 (Ocean Avenue) over the Shark River in the Boroughs of Belmar and Avon-By-The-Sea, Monmouth County, New Jersey. The work to be performed under this Contract consists of Priority Repairs to Bridge W-43. The Proposed improvements entail the removal of the existing fender system which protects the two bascule piers, and the installation of a new timber fender system and navigational lighting system. The contract also provides for other incidental and necessary work shown on the Construction Plans entitled "Bridge W-43 Fender Replacement on County Route 18 (Ocean Avenue) over the Shark River in the Boroughs of Belmar and Avon-By-The-Sea, Monmouth County", dated October 2023, and as further described herein. To assist the interested Consultants become familiar with the project, construction plans, and specifications will be either shared via wetransfer.com or a CD copy can be mailed to the Consultant at their expense for review until the Qualification Statement submission due date. Please note that construction of this project will be paid for with Monmouth County funds and invoicing will follow County of Monmouth standards.
