Stormwater Inlet Replacement Peoria Playhouse Children's Museum The Peoria Park District is soliciting written quotes for the removal and replacement of a stormwater inlet in the East parking lot of the Peoria Playhouse Children's Museum. Please note that the prevailing rate of wage including benefits for the Peoria area shall be paid for each craft or type of worker needed to execute this project or perform this work as required by the State of Illinois Department of Labor. All work performed in connection with this project shall be in compliance with the requirements of all applicable local, state, and federal laws, regulations, rules, and national building codes. Contractor is responsible for field verifying all existing conditions. Please contact Mike Friberg at 309-686-3386 if you have any questions. The project site can be viewed during normal business hours. No appointment is necessary. General Description and Scope of Work: A. General: The intent of this project is to remove and replace the existing stormwater inlet and surrounding concrete and curbing. B. Schedule: Work must be completed September 6, 2024. Award is intended for May 9, 2024. C. Materials: 1. Precast concrete inlet: City of Peoria G1 2. Frame and Grate: Neenah R-3246A with Type 'R' (diagonal) grate. 3. Concrete: 4,000 psi air entrained. 4. Flowable Fill (CLSM): 30 psi mix. D. Execution: 1. Verify all elevations and measurements before ordering precast elements. 2. Call JULIE before you dig! 3. Place fencing (orange snow fence is acceptable) at project perimeter. This area can be extremely busy with public use. No open excavations may be left when contractor is not on site. Excavations must be covered, and fencing maintained. 4. Sawcut pavement and curbing at removal limits. 5. Excavate and remove existing inlet. 6. Place new precast inlet level and to elevations and slopes noted in details. Grout or mortar existing outlet and inlet pipes. DO NOT USE PIECES OF CONCRETE BLOCKS TO FILL GAPS AROUND PIPES. 7. Backfill and compact. Utilize flowable fill for backfill under pavement. 8. Form and place concrete curbing, gutter pan, and inlet apron. 9. Place HMA patching and compact. 10. Clean soil, refuse, and debris from pavement. Administrative Requirements We are required by State law and Park Board policy to request the following to be submitted with quote. Unfortunately, Quotes submitted without this information are considered incomplete and ineligible for award. All forms noted in this section are available from online. If needed, these forms may be faxed or emailed to you for your use. 1. Company Ownership Certification - To comply with Illinois Public Act 102-265, the Park District must collect the information on this form. 2. EEO Certification - A completed Certificate of Equal Employment Opportunity Compliance must be submitted with quotes. 3. Workforce Profile - A completed Workforce Profile must be submitted. 4. W-9 Taxpayer Identification Form: Bidder must complete and sign the W-9 form. 5. Compliance of Listed Provisions: Bidder must sign form. Required forms after Award is made before work can begin: 1. Insurance Requirements: A Certificate of Insurance and Endorsement with a minimum of one million ($1,000,000) of liability coverage, naming the Peoria Park District as Additional Insured, will be required, before work begins. Verification that all employees who will be working at the site are currently covered by Workers Compensation Insurance will also be required. Forms to be submitted with project closeout and pay requests: 1. Certified Payroll - Prevailing wage is required for this job. Please Note: Illinois State Law has changed. As a Contractor on a public works project, Contractor must submit certified payroll directly to the Illinois Department of Labor. After submitting certified payroll directly to the Illinois Department of Labor, Contractor will receive a PDF proof of submittal. A copy of this PDF proof of submittal is required with pay applications to Owner.
