Reference Number 0000351393 The City of Madison Heights is looking for a qualified firm (hereinafter Contractor) to surface, lay out, and line two basketball courts in a 60'x60' area. The area will be new concrete, with a broom finish. Contractor is also requested to prepare and paint or otherwise surface a set of adjacent concrete steps (bleachers). Scope of Work: The proposal shall provide for all labor, materials, tools and equipment as required providing the following: (1) Project Requirements: a. Any necessary preparation of the surface, including any necessary joint fill or leveling. b. Application of a textured sport surface appropriate for unsheltered outdoor conditions and use. c. Layout and application of all play lines appropriate for a basketball court. d. Installation of basketball hoops (post in ground with concrete foundation); basketball hoops to be provided by City. (2) Project Oversight: a. Project Manager: The contractor will provide one (1) person on site as the Project Manager and primary point-of-contact. This project manager will handle all project coordination with the Citys point of contact, as well as any issues which may arise during the project. Further, the Project Manager will complete the final site visit and inspection with City personnel. (3) Construction Process: a. The Contractor shall be responsible for all signage, barricades, or other devices to provide for a safe and visible construction zone while work is being performed. At no time shall an open hole or trench be left unattended. b. Any ruts or settlement created by construction equipment shall be filled to the surrounding grade with top soil, with grass seed raked in to restore the turf. (4) Project Completion: a. The concrete is anticipated to be complete the week of May 6. The City understands that there may be a necessary amount of cure time (30 days) before a surface can be applied to new concrete. Project must be completed and invoiced by June 30 unless otherwise justified in Contractors proposal.
