Only contractors who have been issued a prequalification certificate by the sc department of transportation will be allowed to submit bids. Unless otherwise indicated in the advertisement, no proposal will be considered unless accompanied by a bid bond in the amount of 5% of the Approximate bid amount, written by a surety satisfactory to the Department, properly executed in South Carolina Department of Transportation Form 674 (furnished by the Department in each proposal). If an electronic bond is utilized, the Bond ID number must be provided on the Surety 2000 or SurePath Network electronic bond form. No other form of proposal guaranty will be accepted. The south Carolina department of transportation reserves the right to reject any or all proposals and waive any technicalities. SC DOT - 040 - S-195 (New Cut Church Rd) Bridge Replacement Over Camp Creek The following claims procedure covers all claims for additional time or compensation arising under this contract. The claims procedure is non-binding and is a condition precedent to litigation or any other form of dispute resolution. All communications, testimony, and all documents prepared for this procedure by either party from the time of filing the CERTIFICATE OF CLAIM, per section 105.16.8 of the 2007 Standard of Specifications for Highway Construction, to the conclusion of the procedure shall be deemed to be settlement negotiations and not admissible in litigation or any other dispute resolution procedure. If at the conclusion of this procedure the claim has not been resolved, litigation may be pursued through the South Carolina Circuit Court for Richland County. Contractor waives all rights to a jury trial and agrees that all litigation matters shall be heard non-jury and venue for any action shall be in the South Carolina Circuit Court for Richland County. Furnish all required labor, equipment, and materials and perform all operations necessary for installing anchors and dowels in concrete using an adhesive bonding system in accordance with the details shown on the Plans and with the requirements of this Specification. Provide a material system specifically intended for use in structural applications for bonding anchors and dowels to hardened concrete. Limit applications to anchors and dowels installed in horizontal, vertical, and downwardly inclined positions. Do not use adhesive anchors in overhead or upwardly inclined installations. Furnish all required labor, equipment, and materials and perform all operations necessary for installing the elastomeric concrete in accordance with the details shown on the Plans and with the requirements of this Specification. F&ME performed a geotechnical subsurface exploration and laboratory testing for the bridge and roadway portion of the project. The South Carolina Department of Transportation (SCDOT) request and scope for the geotechnical subsurface investigation was issued on June 12, 2019. A subsequent laboratory testing scope was issued on August 23, 2019. Copies of these requests can be found in Section 8 of the Appendix of this report. The field investigation consisted of Soil Test Borings (STB) with Standard Penetration Testing (SPT), bulk soil sampling, and auger borings with Shelby Tube sampling. Laboratory testing was performed on soil samples collected from the soil test borings, bulk samples, and undisturbed samples. All exploration methods and laboratory procedures were conducted in general accordance with the current American Association of State Highway and Transportation Officials (AASHTO), American Society of Testing and Materials (ASTM) Standards, and the SCDOT Geotechnical Design Manual (GDM). This report was prepared in general accordance with the 2019 SCDOT Geotechnical Design Manual (GDM), Version 2.0. At the request of F&ME Consultants, GRL Engineers conducted SPT energy measurements in general accordance with ASTM D4633-10 during SPT sampling at a site near the intersection of Lowrys Highway and Hardin Strait Rd in Chester County, South Carolina. Energy measurements on the three rigs were taken during five sampling events at five-foot intervals in dummy soil borings. The starting depth was 28.5 feet for each of the rigs, and ending depth was 50 feet below the existing ground surface. The SPT samples were driven for a total of three, 6 inch increments, or 1.5 feet, and the blow counts for each increment were recorded.
