This project will be a phased construction expansion and repurposing of current space project of approximately 35,000 SF and a total budget of $30,000,000. Copper Queen Community Hospital, located in Bisbee, Arizona, has a rich heritage, dating back to the 1880s when it operated as part of the mining community, as such is key to the history of Bisbee. This 14-bed small acute care hospital strives to provide convenient, affordable health care to the area it serves for over 125 years. The hospital provides a broad range of inpatient and outpatient services, including acute care; outpatient surgery; 24-hour emergency services; cardio-pulmonary services; rehabilitation services; home health care; an occupational medicine department; a full-service laboratory; and diagnostic imaging which includes a state-of-the-art fluoroscopy unit, flat plate basic radiology unit, Dexa-Scan bone density unit, spiral CT scanner, mammography services, ultrasound, on-site experienced and professional radiologists, and teleradiology for speedy diagnostic capability. The local medical staff includes 23 physicians which 12 are local fully active physicians and 11 are consultants. You can find the most current information at online In order to allow for an adequate evaluation, CQCH requires an offer in response to the solicitation to be valid and irrevocable for 90 days after the opening time and date. It is expected that the award for this contract will be made in 45 days. Firm shall maintain in current status all federal, state and local licenses, bonds and permits required for the operation of the business conducted by Firm. Firm shall remain fully informed of and in compliance with all ordinances and regulations pertaining to the lawful provision of services under the Contract. CQCH reserves the right to stop work and/or cancel the contract of any Contractor whose license(s) expire, lapse, are suspended or terminated. Performance security shall be in the form of a performance bond, certified check, or cashier's check. This security must be in the possession of the CQCH Business Office within the time specified or ten (10) days after agreement of GMP. If the contractor fails to execute the security document as required, the contractor may be found in default and the contract terminated by CQCH. In case of default, CQCH reserves all legal rights to rectify matter. All performance bonds must be executed on forms substantially equivalent to Performance Bond format attached to this RFQ. The successful CM@Risk contractor shall be required to furnish a Payment Bond equal to 100% of the GMP. The bond shall be submitted within 10 days of the establishment of the GMP. The surety will be in the form of a bond, cashier's check, certified check, or money order. All payment bonds shall be executed on forms substantially equivalent to the sample enclosed with this RFQ. Personal and company checks are not acceptable unless they are certified.
