GBHC seeks a vendor to complete an all-inclusive milling and paving contract to resurface necessary roads. Contract term: Approximately 60 days Location: Greater Binghamton Health Center All written questions regarding this solicitation must be sent to Leonard.Nauta@omh.ny.gov via e-mail only by June 27th, 2024, 3PM EST. Questions and answers will be accumulated by Leonard Nauta and sent to all potential bidders by July 3rd, 2024. Questions and Answers will also be posted to the New York State Contract Reporter Ad. Anticipated Work Start Date: July 29th, 2024 Any change in scope of work, services, products, and cost must be approved in writing by GBHC prior to the changes being made No work can begin without prior written approval from GBHC No work can begin without a preconstruction safety plan being completed and on file No work can begin until proof of required insurances have been verified by GBHC Business Office Contractor training may be required before work can begin Healthcare screenings and required PPE (Masks) will be determined by the rules established by GBHC, OMH central office and other agencies and departments Proof of specialized training, certificates, licenses may be required by all workers Prevailing wages apply to this job, please consult attached Wage Schedule Certified payroll will be required for invoicing Proof of cost of materials will be needed for invoicing. Percentage of mark up will be capped at 10 percent GBHC will not be responsible for shipping, delivery, and handling fees for any items GBHC will not receive items purchased by the contractor Work hours are Monday through Friday 7:30 am to 4:00 pm. These hours may be adjusted to meet the facility operational needs and may vary depending on where work is taking place. All NYS recognized legal holidays will be observed as non work days, a variance can be made for this, but will be required to be in writing and agreeable by all parties All contractors must sign in and out daily at a place to be determined by the facility. If GBHC allows onsite storage of tools, materials, equipment, etc. GBHC assumes no responsibility for said items and said items must be securely kept at all times SDS sheets may be required for all products, items brought onto the campus prior to said items arriving on the campus No tools, materials or equipment will be allowed to be kept in the wok zone, without prior written authorizations from GBHC The safety department must be notified a minimum of 30 minutes prior to the finish time of every workday to inspect the area Any waste, debris, garbage, etc, produced during the job will be the responsibility of the contractor to remove from the job site and dispose of properly and legally at the designated dumping area provided by Facility. The location of the dumpster and or garbage collection area will be determined by GBHC, please reference provided campus site maps. The work area must be kept clean, orderly, and safe at all times Project 1 will start within 30 days of bid award, unless a mutually agreed upon change is established Project 1 will be completed within 30 days of Project 1 start date, unless a mutually agreed upon change is established Project 2 will start within 7 days of completion of Project 1, unless a mutually agreed upon change is established Project 2 will be completed within 30 days of Project 2 start date, unless a mutually agreed upon change is established If any other information or documentation is provided that conflicts with the information contained herein, this IFB will take precedence.
