INTRODUCTION The Subcontractor shall furnish all properly qualified management, supervision, personnel, materials, supplies, tools, transportation, and equipment with appropriate accessories (except as provided by FRA) and shall plan, schedule, coordinate and assure annual inspections for roofs on site. The Subcontractor shall perform roof inspection services of the roof trade covered by the Service Contract Act. SCOPE OF WORK The subcontractor shall furnish all labor, equipment, material, transportation, supervision, and oversight to perform a yearly visually inspection on 517 roofs. It is critical that subcontractor has the resources to inspect all roofs in a calendar year. Inspections should include cleaning any debris from the roof drains and noting if the gutters need cleaning. Number of roofs may increase or decrease over time of contract. Some buildings have multiple roofs. The site covers 6800 acres. We are using an application on an IOS device, phone or tablet, to manage Inspection Work Orders for the roof inspections and enter some of the data directly into centralized maintenance management system. The contractor will be required to utilize this mobile application. We will supply the application and assist the inspector in the use of this procedure[MABx11] [RBSx02] . We will have the capacity to support Android devices during the life time of the contract. The inspector will supply priority for the repairs, estimated cost of repairs, remaining years of life for the roof. It will be enterable with the application. Photos and comments on what the inspection found will be entered via the application. A formalized report summarizing the data in the above bullet point upon completion of all roof inspections. Photos will be required of the entire roof and a photo for each problem found that needs a repair scheduled. Photos should also be used to show the condition of a roof requiring replacement. Photos should be taken and uploaded to the PM work order using the application on the iOS device. All work requested by Fermilab to be performed by the Subcontractor (hereinafter defined to include the furnishing of labor, equipment, material, transportation and supervision) shall be authorized by Fermilab written issuance to the Subcontractor of a Work Order. The Work Order shall define the location of the roof to be inspected and special conditions or restrictions that may exist at the work site. The Work Order will be emailed to the roof inspection subcontractor. Except as otherwise provided in this Subcontract, the Subcontractor shall not be paid for any work which is not authorized by a Work Order. It is understood that more than one Work Order can be issued by Fermilab which requires work to be performed at differing site locations during the same period of time. The Subcontractor shall be responsible for scheduling and managing all such assigned work to assure timely utilization is being made of all available labor, equipment, material, and supplies assigned to the work. The scheduling of assigned work will be coordinated by the ISD FM planning and scheduling representative, designated service coordinator / area facility manager and the subcontractor. Work Orders shall be issued by a Fermilab's ISD Facility Manager planning and scheduling representative. Overtime will not be allowed unless specifically authorized by the Fermilab representative or his designee overseeing the contract. The contractor should schedule his visit with the designated service coordinator prior to coming on site. During normal working hours of 7:00 am to 3:30 pm, the serviceman will notify the service coordinator or his designee that he is on site. The Subcontractor is required to keep track of their employee's Fermilab training and schedule new or updated training as needed. They will provide methodology and process on how training will be scheduled, executed, documented and kept current for current and new employees. The Subcontractor shall be responsible for maintaining satisfactory standards of employee competency, conduct, and integrity and shall be responsible for taking such disciplinary action with respect to its employees as may be necessary The Subcontractor shall be responsible for daily cleanup of his work areas and remove all generated debris. Specialized Software / Device Requirements / Description We are using an application on an IOS device to enter the service data directly into centralized maintenance management system. We will supply the application, provide training and assistance in the use of this new procedure PRE-PROPOSAL MEETING/SITE VISIT A Pre-Proposal Meeting and Site Visit are scheduled for 2:00 P.M., Central Time, June 17, 2024, at Fermilab Site 38, Batavia, IL. The meeting provides an opportunity to discuss and clarify the RFP. However, nothing said or represented in the meeting shall be deemed to modify RFP requirements unless followed by a written amendment. Offerors are requested to submit questions relative to the RFP in advance of the meeting so the Procurement Specialist may prepare responses. Offerors' participation at this meeting is strongly recommended. Except those already having a badge, anyone attending the Pre-Proposal Meeting and Site Visit is required to fill out an Access Request form (see link below) at least seven (7) days in advance of the scheduled date and preferably be a U.S. Citizen. Onsite access approvals are dependent on citizenship status and the location(s) being visited. Upon request approval, the individual will receive an informal invitation letter with a QR code, which is required to be presented at the security gate, otherwise, access to Fermilab will be denied. When checking in at the Security Gate House, the individual will be required to present a Valid Driver's License (Real ID) and the informal invitation letter with the QR code, and the Visitor and Business Page 3 of 7 Visitor Attestation Form. If the Driver's License is not Real ID compliant, the appropriate REAL ID documents (https://www.tsa.gov/travel/security-screening/identification) are required. Depending upon the Community Covid Level, masks may be required to be worn. If face covering is required, it is recommended the individual wear a KN-95 or N-95 mask. Unfiltered cloth masks are not permitted to be worn alone but can be worn over a Level 2 medical mask (double mask). Please note that when entering Fermilab: o No weapons, firearms, or ammunition (regardless of concealed and carry permit) are allowed on-site. o No explosives or any other apparatus or material hazardous to the public or property are allowed on site. o No possession or illegal use of controlled substances or intoxicants or being under their influence are allowable. o Illinois Rules of the Road are to be followed while driving on Fermilab's premises.
