Reference Number 0000272213 Question Acceptance Deadline 2024/06/25 03:00:00 PM EDT Grand River Hospital is Seeking the Services of Experienced Plumbing Vendors to Provide Complete Plumbing Services Including but Not Limited to Provide Plumbing Services at Grh, Including Inspection and Repair Work From a Licensed Tradesman and/or an Apprentice/labourer. Grh Also Requires That Vendors Must Have the Ability to Respond to a Service Call 24 Hours a Day, 7 Days a Week and Be on Site Within One Hour or Less From the Time the Service Call is Placed. The Hospital Intends to Select a Total of Three (3) Plumbing Vendor(S), Based on Service Availability and Pricing. The Review of the Service Provider(S) Capabilities and Qualifications Will Include a Review of the Respondent's Compliance With the Specifications, Terms and Conditions Included Herein, Including an Assessment of the Degree of Obligations and Responsibilities That the Respondent is Prepared to Assume in Respect of the Supply, Work or Business Described Herein. The Service Provider(S) Are Requested to Respond and Provide the Required Information in Schedule 1 "Vendor Evaluation Criteria" (Evaluation Matrix a), and Schedule 2 "Financial Submission" (Evaluation Matrix B). Vendors Are Requested to Submit Requested Documents, Vendors Who Fail to Provide the Documents May Be Disqualified 1. Prequalification Proposals Are Requested From Interested Plumbing Contractors. Firms Submitting Proposals Shall Have Recently Completed Health Care Related Projects of a Similar Type and Scope to This Project and Shall Be Able to Provide References for Those Projects. 2. The Information Submitted by Proponents is Understood to Be Confidential. This Confidence Will Be Respected by the Owner and Consultant Who Require Same to Evaluate Proponent Contractors. 3. The Use of Bid Depository is Not Anticipated. 4. Only Prequalified Plumbing Contractors Nominated in the Bid Documents, Will Be Permitted to Submit Second Stage Bids. 5. The Results of This Prequalification Process May Be Used to Establish the Bidders List for Future Work at This Facility. 6. The Established Bidders List Will Remain in Effect for a Period of Two (2) Years With the Option to Extend the Agreement for Three (3) One (1) Year Periods. 7. Should There Be Material Changes to the Circumstances of Any of the Prequalified Bidders, the Owner Reserves the Right to Modify the List Accordingly.
