The Ministry of Health (Ministry) is seeking proposals to develop and perform an evaluation of the Long-Term Care at Home (LTC@Home) Program. Program monitoring and evaluation is needed to measure program outcomes, identify opportunities for improvement, and inform provincial program expansion planning. The selected contractor will be responsible for data collection and analysis at all pilot program locations and will provide a complete evaluation report including evidence-based recommendations for program improvement. Travel may be necessary to perform site visits and in-person meetings to Long Term Care Homes located within Island Health and Fraser Health authority regions at the Ministry's request.
