Scope of Work Perform scheduled and emergency distribution system repair and new water main installation services including restoration for the Dix Hills Water District in accordance with the requirements stipulated herein and all prevailing federal, state, and local safety and labor regulations. Services may include but not be limited to water main break repair, installation and repair of service lines, valve replacement, hydrant replacement, and water main installation, replacement and pavement repairs. Work will be performed on an as-needed basis when authorized by the District in writing. The District Superintendent will fax a work order outlining the location and nature of work to be performed. Scheduled distribution system installation is nonemergency work and will be scheduled by the District 48 hours in advance of the start date. Emergency service shall be provided to the District on a request basis. For emergency service, the Contractor's work crew shall report to the site in need of service within one (1) hour after notification from the District. Notification from the District will be via phone call (for emergency work) and a fax transmission of the work order for the service requested. A copy of the work order form is included in Appendix A. Contractor will be required to maintain the proper bonds and insurances as specified herein. Contractor shall guarantee workmanship and materials for a period of one (1) year upon completion of the work for distribution system installation, repairs and road restoration work. The Contractor shall arrange for all mark outs related to the work. Contractor shall confirm all mark-outs prior to starting any work. A proposal submitted by a bidder who is not in full compliance with the provisions of the Huntington Town Code at the time of submission will be denied. Work areas may be throughout the distribution system, including plant and water storage tank sites of the Dix Hills Water District, Dix Hills, New York The right is reserved by the Town to waive any informalities in, to reject any or all bids submitted, or to accept the bid and award the contract to the lowest, responsive, responsible, formal Bidder, in the best interests of the Town, pursuant to Section 103 of the General Municipal Law. Question Deadline 07/03/2024 at 4:30 PM ET: Questions, requests for information or interpretations concerning the drawings or specifications, or any aspect of the project must be addressed in writing to the Director of Purchasing, Town of Huntington, Town Hall, 100 Main Street, Room 209, Huntington, New York 11743, email at purchasing@huntingtonny.gov or Fax # (631) 351-2833 and to be given any consideration, must be received by the close of business, 4:30 p.m. EST, at least five (5) business days prior to the date fixed for the opening of bids. The right is reserved to waive any informalities in, to reject any or all bids submitted, or to accept the bid and award the Contract to the lowest responsible bidder, therefore pursuant to Section 102 of the General Municipal Law. Award of Contract: If written notice of the acceptance of this proposal is mailed, faxed, or delivered by the OWNER to the accepted BIDDER within forty-five (45) days after the date of opening of Proposals, or any time thereafter before the Proposal is withdrawn, the accepted Bidder, within five (5) calendar days after the date of mailing, faxing, or delivering of such Notice of Acceptance, shall attend at the office of the Town Attorney, Town Hall, 100 Main Street, Huntington, New York 11743, with sureties offered by him and there sign the contract for the work in quadruplicate. The accepted bidder, upon his failure or refusal to execute the Contract and bond required within five (5) calendar days after the date of mailing, telegraphing, or delivering of such Notice of Acceptance of his proposal, agrees to forfeit to the TOWN as liquidated damages for such failure or refusal, the security deposited with his proposal. The Town reserves the right to award future contracts/purchase orders to the next low bidder in the event of default or cancellation of a previously awarded contract/purchase order. Each bid must be accompanied by a bid bond or certified check of the BIDDER payable to the TOWN OF HUNTINGTON in an amount not less than five percent (5%) of the amount of the TOTAL BID PRICE. Checks or bid bonds will be returned to all except to the three lowest formal bidders within ten (10) days after the formal opening of bids. The bidder understands that the term of this Contract shall be effective for one year commencing upon execution of the contract but not prior to November 1, 2024 After the initial one (1) year contract period, two (2) one-year contract extensions are available by the mutual consent of the Town of Huntington and the Contractor in accordance with the terms and conditions acceptable by the Town Attorney The total amount of the Contract shall be as determined by the Town at the time of award and as may be further changed by the Town as the work progresses. The Contract will be awarded to the Contractor who is determined to be the LOWEST RESPONSIVE, RESPONSIBLE BIDDER. The term of this Contract will commence upon execution and will continue through October 31, 2025. After the initial one (1) year contract period, two one-year contract extensions are available by mutual consent of the Town of Huntington and the CONTRACTOR in accordance with the terms and conditions acceptable to the Town Attorney. The Town reserves the right to reject any request for assignment for an entity that appears on the prohibited entity list prior to the award of a contract, and to pursue a responsibility review with respect to any entity that is awarded a contract and appears on the prohibited entities list after contract award.
