(330.3) Grade existing pulverized asphalt pavement to a width of twenty-six (26) feet. Unit cost for Item 001-Pulverization of Existing Asphalt Pavement Extended cost for Item 001-Pulverization of Existing Asphalt Pavement 002-Sawing: Sawcut existing asphalt for a clean butt joint. Sixty-Five (65') Feet Approximate LF (West End) 4th Street Seventy-Three (73) Feet Approximate LF (East End) County Road EE One Hundred & Thirty-Eight (138') Feet Total Approximate LF Asphalt Sawing (690.3.3) Unit cost of item 002-Sawing of Existing Asphalt Extended cost of item 002-Sawing of Existing Asphalt 003-Base Course: Approximately 2,400 Tons Furnish and install three (3) inches of WISDOT Spec. 1-1/4" BAD Material to a width of twenty-six (26') feet. Fine Grade WISDOT Spec. 1-1/4" BAD Material Furnish and Install Water for Dust Control and Compaction. Furnish and Complete Compaction on WISDOT Spec. 1-1/4" BAD Material (301.3.4.2) Unit cost of Item 003-Base Course Extended cost of Item 003-Base Course 004-HMA Pavement: Approximately 1.400 Tons Furnish and Install WISDOT Spec. Type L Asphalt Mix-Twenty (20) feet wide. Compact WISDOT Spec. Type L. Extended Cost for Item 006-Driveway(s)/Field Entrance(s)-Base Course 007-Traffic Control/Signage LS -Furnish and maintain all traffic control, signage, flags, flag-persons, barricades and lights where required, to protect the safety of the traveling public and workers, in accordance with the Manual on Uniform Traffic Control Devices Extended Cost for Item 007 Traffic Control/Signage 008- Corrugated Metal Pipe LF Remove three existing corrugated metal pipes. Install One, eighteen (18") inch by forty (40') feet corrugated metal pipe. Install One eighteen (18") inch by twenty-eight (28') foot corrugated metal pipe. Install One, eighteen (18") inch by twenty-eight (28') foot corrugated metal pipe. Maintain correct pipe cover according to CMP Manufacturer. Unit Cost for Item 008-Corrugated Metal Pipe Extended Cost for Item 008- Corrugated Metal Pipe 009-Mobilization LS Extended Cost for Item 009-Mobilization Total Project Cost Specials/Other. Utilities: Contractor is responsible for all Utility Locates and Relocates. Liability Insurance: The contractor shall furnish and deliver to the owner a certificate of insurance for worker's compensation and an umbrella certificate of liability and property insurance before beginning work, and shall notify the owner immediately of any cancellation or change in insurance coverage. Engineers Certification: An Engineer's Certification is required pursuant to s. 86.31(2)(e), Wis. Stats. The certification must include the signature and stamp of a registered professional engineer, stating that the design, plans, and specifications were assembled with a 10-year design life criteria for the improvement, and that the design and construction were accomplished using current WisDOT specifications pursuant to Ch. Trans 206.03(10) (b). Refer to the Local Roads Improvement Program (LRIP) Guidelines and Requirements, Appendix R for a sample of an Engineer's Certification. Performance Bond: A Performance Bond in the amount of 100 percent of the total bid price will be required of the successful bidder in accordance to Wis. Stat. 779.14(1m)(d)3.
