Reference Number : 0000357563 As part of the City of Monroe Preventative Maintenance Program, quotes are requested to clean / jet and CCTV approximately 5,565 lineal feet of sanitary sewer line (ranging between 27" and 36") within the City of Monroe and Frenchtown Charter Township (see attached maps). Scope: Cleaning / jetting / root cutting of the sanitary sewer lines prior to the video inspection of the line are required. Some pipe may have high flows, require heavy cleaning, and have roots that need to be cut for CCTV. 90% of the pipe shall be free of solids, sediment, & debris. Bypass pumping may be required & shall be included in the bid prices. Video inspection of the cleaned sanitary sewer line, noting the condition of pipe, footage, taps, defects and manhole numbers in accordance with the most current NASSCO PACP standard, at least 75% of the pipe wall shall be provided at all times. Bid price shall include any necessary traffic control and or permits. Notes: City of Monroe will provide all water needed for cleaning. Contractor must have at least a minimum of 3 years of experience in the field of sewer pipeline inspection by means of CCTV and have the required capital, organization, and equipment to perform the work in accordance with the terms and provisions of this project. Contractor shall have successfully carried out work similar in nature, scope and value to the work and demonstrate that within the past 3 consecutive years prior to the bid, the Contractor has successfully completed over 500,000 feet of previous CCTV on sewers for condition assessment purposes. Some pipes are in difficult locations, a site visit is highly recommended, no additional costs will be provided for providing accessing to manholes and or accessing the pipe. Coordination of daily work schedules with Collection System Supervisor is mandatory. City of Monroe will provide a dump site for all debris from the sanitary sewer lines (WWTP 2205 E. Front St, Monroe, MI 48161). Dewatering of trucks is required before dumping debris at dumpsite. City of Monroe will locate any manholes, adjust manholes (as needed) and provide manhole numbers (if needed). GIS shapefiles can be provided upon request. All inspections and reports provided to the City of Monroe are to be completed in accordance with the most current NASSCO PACP standard. Confined Space Safe Entry practices are to be followed. All work is to be completed and reports / logs including all video inspection (flash drive, portable drives, etc) shall be provided to the City of Monroe by 12-31-2024. Liquidated damages shall be assessed in the amount of $75.00 per day if contract work is not completed by deadline. Work hours should be generally Monday through Friday, 7:00 am until 5:00 pm, but open to a modified schedule. The contractor's bid shall be based on a price per lineal foot ($/LFT) by the size of pipe as shown in the table below. The price shall include all design, purchases, delivery, material, labor, equipment, construction equipment, transportation, permitting, inspections, etc necessary to complete the contract. It will be the contractor's responsibility to provide for all safety related equipment necessary to perform the work safely, to adhere to MIOSHA standards, and to adhere to the systems policies and procedures throughout the contract. The contractor shall be responsible for any damages to the system that they cause during the work. Repairs shall be made to pre-construction or better condition. The contractor shall observe and comply with all federal, state and local codes, ordinances, laws and regulations in force related to the location. Contractor shall provide liability insurance. The Contractor shall list the City as an additional insured under the Contractor's liability policy. Each subcontractor shall be required to maintain the same forms of liability insurance referred to above and all subcontracts shall contain the same complete indemnification provisions as set forth in this contract, indemnifying the City of Monroe, its agents, officials and employees.
